Learning Objectives

Attendees will gain understanding of the Best Practice Spotlight Organization including a brief review of history and current status, as well as the Best Practice Spotlight Organization in the context of primary care, specifically applied to the Guelph Family Health Team and our Medical Home Model. Learning objectives include knowledge and skills related to implementation science and translating theory to practice as well as how we have applied this within our organization. Emphasis will be placed on work to date, including successes and challenges highlighting dissemination strategies, staff engagement, levels of leadership, patient-centered care and integration with organizational strategic plans and priorities.

Summary/Abstract

Evidence-based practice in Healthcare often leads to both enhanced quality improvement measures and improved patient safety. This is the aim of most clinicians when providing care to their patients, however there are a number of barriers that prevent patients from receiving the care they deserve. Possible barriers related to providing evidence-based care to patients include difficulties in developing evidence-based clinical policy, poor access to best evidence, organizational barriers, and ineffectual continual education programs.  To help support and enhance evidence-based practice within our organization, the Guelph FHT has partnered with the Registered Nurses Association of Ontario as a Best Practice Spotlight Organization. While in our 2nd year of this partnership, we have learned the needed skills to create a successful implementation strategy and applied implementation science to help support staff improve knowledge and skills related to their clinical work. Using the tools of implementation science, we have identified evidence-based recommendations based on some of the gaps in the care we provide. In the first year of being a BPSO pre-designate, we have supported the successful implementation of 2 Best Practice Guidelines, with 3 upcoming in year 2.   Through alignment with FHT strategic plans and organizational priorities, we have achieved collaboration of both interdisciplinary team members as well as projects across the FHT; this has created high performing teams, enabling competency based practice that provides whole care to our patients and communities.
