Primary care: The Best Care Takes a Team

For most Ontarians, primary care providers are the entry point to the health care system. When patients are sick or hurt, they turn to them first.  And for the best care, it takes a team.

As the government moves forward with primary care reform to reduce complexity, team-based primary care offers the best model for:

  • Helping end hallway health care by avoiding costly emergency room visits, which includes providing much needed supports for individuals dealing with mental health and addictions issues;
  • Ensuring people in all parts of the province – especially in rural and northern communities – have reliable and equitable access to a primary care team; and
  • Providing care that improves the patient experience while offering the best value for money in health spending. 

Executive directors, board chairs, frontline providers and patients are encouraged to meet with their MPP to communicate the value of team-based primary care during this time of significant health care transformation.
