Sharbot Lake Family Health Team is seeking an Occupational Therapist to work as part of an energetic and committed inter-professional team. The Occupational Therapist will work to their full scope in a collaborative practice alongside physicians, nurses, and other health care professionals to provide our patients with individualized, effective, timely, and efficient care. 

Sharbot Lake Family Health Team is a multidisciplinary team of health care professionals who provide primary health care services to residents in the communities of Central Frontenac as well as portions of North and South Frontenac. As an Occupational Therapist with our team, you will work both independently and in collaboration with physicians and other health professionals to provide health promotion and chronic disease management. 
This is a great opportunity to make a real difference in a community and be able to enjoy the pace of a family health clinic. This position is part time out of Sharbot Lake 0.4 FTE. We have proposed a FHT expansion to the town of Perth, which if approved, would provide a full-time position split between the two clinic sites.

Position Summary
As a member of the multidisciplinary team, the Occupational Therapist’s role is to provide thorough care and support to patients and their families or caregivers, including cognitive assessments, physical evaluations, and assistance navigating community resources. This position requires travel and home visits and potentially work at two FHT sites, so the successful applicant must be able to travel and hold a valid driver’s license.

Education, Training and Qualities:

  • Registered with the College of Occupational Therapists of Ontario (COTO) and a member of a Professional Association/Society
  • Have at least two years related experience.
  • Possess enthusiasm for working with patients across a broad spectrum of ages and challenges.
  • Demonstrated experience in community and/or primary care medicine in an interdisciplinary model or care.
  • Demonstrated expertise in identifying and addressing the needs of people at risk for functional decline, assessment of daily living activities and falls risk.
  • Creative and have realistic ideas about how to improve the lives of patients.
  • Ability and desire to develop new skills and evolve with a continuous improvement focus.
  • Leadership, organization, research, evaluation, time management, communication, critical thinking, and interpersonal skills
  • Ability to work independently and within a team environment,
  • Proficiency with computers and electronic medical record systems. 


Key Responsibilities:

  • Develop long term therapeutic relationships with patients, conduct assessments using recommended preventative interventions and risk assessment, create patient learning plans.
  • Liaise with other team members to achieve optimal outcomes for the patient.
  • Employ critical thinking, evidence-based medicine, and best practice as frameworks for practice.
  • Document observations, conversations, interventions, and collaboration on patient care in the patent record.
  • Participate in community health initiatives such as education and building community networks with other organizations involved in improving community health.

  • This is currently a part-time (0.4 FTE) permanent position.

  • Compensation is commensurate with experience and in keeping with the amount set by the Ministry of Health for Occupational Therapists.
