Job Description:

Prime Care Family Health Team is looking to recruit a dedicated permanent part-time Mental Health Youth Counselor to join our team. The Youth Counselor will provide primary health care functions and work in collaboration with Prime Care FHT’s full Interprofessional Healthcare Provider (IHP) team. The Social Worker is responsible for completing youth and family assessments and will work collaboratively with a multidisciplinary team to develop service plan goals and provide supportive intervention for youth and families in a manner consistent with best practices. 

Requirements

  • Graduate degree in Masters of Social Work from a recognized university
  • Minimum 3 years combined education and experience in primary care setting
  • Experience related to community-based youth mental health is preferred.
  • Membership with corresponding College, as applicable, and proof of professional liability insurance
  • Good understanding of relevant provincial and local legislation and government policies, procedures and practices pertaining to services provided by Social Workers
  • Knowledge of therapeutic models of individual, youth, group and family counseling and the ability to apply same
  • Expertise in brief treatment approaches and Cognitive Behaviour Therapy.
  • Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population
  • Knowledge of client-centered philosophy, basic principles and practices of community service delivery and short-term treatment options
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
  • Knowledge of community resources connected to the provision of mental health and addictions, and related services
  • Demonstrates good interpersonal and networking skills
  • Ability to anticipate crises and apply crisis theory during crisis situations as appropriate
  • Demonstrates skills in assessment, program planning, implementation and evaluation, health promotion and community development
  • Ability to work flexible hours (some evening work may be required)
  • Experience with documentation of (EMR), preferably ACCURO

Roles and Responsibilities:

  • Perform client Psychosocial Assessments, Treatment, Psycho-Education and Evaluation.
  • Provide psycho-education to clients through individual or family interchange and provide referral to community resources as required
  • Responsible for making clinical decisions and exercising judgement to ensure best possible care is provided within available resources.
  • Conduct intake assessments and provides counseling services, with particular regard to individual youth and family issues and decisions that impact the health of the patient and family
  • Work within the primary care model of brief therapy, ensuring seamless transition to community resources as appropriate
  • Is knowledgeable of the community and the resources available, especially in the areas of crisis services, mental health, and long term care
  • Assist the referring physician/health care providers with referrals to secondary and tertiary health care centers
  • Build strong relationships with PCFHT Mental Health Team Members.
  • In collaboration with Physicians, Nurse Practitioners and other Interdisciplinary Health Professionals, implement and discuss appropriate individualized care plan with the client/family based on best practices.
  • Identify, analyze and provide appropriate short-term, solution focused treatment and/or referral for youth with mental health and addictions problems.
  • Share results of assessment and intervention with referring physicians and other team clinicians within a specified timeframe.
  • Support patients to manage their clinical plans including healthy choices, healthy lifestyles, appointments, medication as necessary, and self-management of care.
  • Assist patients and providers to navigate the health and community service systems to identify and connect them with appropriate services and supports.
  • Participate in the planning, implementation and evaluation of mental health and addictions programs and services.
  • Contribute to PCFHT Quality Improvement Plan (QIP) in collection of performance measurements of patient outcomes.
  • Participate in regularly scheduled staff meetings in order to exchange ideas, receive mutual support and monitor program outcomes as well as present case studies.
  • Maintain relations with network, related professional organizations and other related service providers.
  • Assist in the training and supervision of students and or/volunteers.
  • Participate in team building, program development, and improvement activities.
  • Comply with established personnel policies, procedures, standards and guidelines for operation
  • Demonstrate knowledge in health promotion and chronic disease management for primary care
  • Collect, compile and provide statistics on programs
