Job Description

The Madawaska Valley Family Health Team provides team-based primary care to the local Barry’s Bay community. Working with the primary care physicians, our professional staff includes nurse practitioner, registered practical nurse, social worker, dietitian and respiratory therapist. Patients are referred to the social worker from the collective practices of ~ 7,000 patients. 

The Social Worker is the primary mental health resource within the family health team. The social worker’s clinical services include therapeutic assessment, interventions, counselling, and supportive liaisons with community resources. 

Key Responsibilities: 

  • Conduct psychosocial assessments and intervention plans for referred patients.
  • Develop short-term therapeutic relationships.
  • Provide a range of therapeutic interventions and counseling on a short-term basis.
  • Management of common mental health problems, ie., anxiety, depression, grief/loss.
  • Maintain accurate and concise confidential documentation in the electronic medical record.
  • Establish and maintain ongoing liaisons with agencies and community resources.

Skills and Qualifications:

  • Masters in Social Work (MSW) or Bachelors in Social Work (BSW) from a recognized university or an equivalent program
  • Member of the OCSWSSW
  • Minimum 3-5 years counseling/therapy experience in a primary care setting preferred
  • Proficient in assessment and psychosocial counseling techniques.
  • Experience with addiction counselling considered an asset.
  • Knowledgeable of the relevant legislation.
