Ontario’s vision for connected primary care depends on more than expanding access. It requires strong relationships between primary care organizations, Ontario Health Teams, community partners, specialists and hospitals that are built around the needs of local communities and the full population of people within them. Join leaders from Southlake Academic Family Health Team, the Northern York South Simcoe Ontario Health Team, and Southlake Health as they share how they are building a neighbourhood-based model of integrated care across 6 unique communities in the Northern York South Simcoe region.
