We represent the members across the full team: 2,700+ physicians, 450+ NPs, 500+nurses, and 1,300+ independent healthcare professionals.

AFHTO has the network and expertise to rapidly deploy primary care solutions across the province with its members, ensuring Ontario meets its goal to attach 2 million patients by 2029 AND to continue to care for the people we already serve

Current Team Statistics

While 25% of Ontarians (4 million) are attached to our 190 member primary care organizations (FHT’s, NPLC’s etc.), only 16% of Ontario’s family physicians work in our member organizations, reflecting the outsized role the teams play in driving attachment and access.

More than half of AFHTO primary care teams operate with a mixed board governance model.

AFHTO MEMBERS BY GOVERNANCE MODEL

  • Mixed Board: The board of directors/voting members are made up of clinicians, members from the community, and members from corporate.
  • Community-Led Board: The board of directors/voting members are purely community members (i.e., no clinician representation or there are clinicians on the board but they are non-voting).
  • Clinician-Led Board: The board directors/voting members are purely clinicians (i.e., no community representation)
ACADEMIC STATUS: Many teams have academic status, serving as essential community-based teaching clinics for the next generation of healthcare professionals.
