Job Title: Quality Improvement (QI) Coach / Specialist
Position Type: 0.2 – 0.5 FTE Part-Time 6 month Contract (Project-Based with possibility of extension) 
Reports To: Director Health System Data, Research and Quality Improvement
Location: Remote with occasional travel in Ontario
Organization: Association of Family Health Teams of Ontario (AFHTO)
Salary: Based on full time salary range $70,000 - $85,000

As many of you are aware the EMR communities of practice (CoP) will be transitioning from AFHTO to OntarioMD effective March 31, 2020.
