Best practice evidence identifies the organization of primary care and primary care leadership as essential in the success of integrated health care systems. With the development of Ontario Health Teams, it is critical that local primary care has organized and sustainable governance models to help support the continued partnership with other health care providers in planning and implementing local strategies for integrated care.
