Job Title: Registered Dietitian – Community Health Planner
Reports to: Manager, Operations and Culture
Status: Permanent Full Time

Summary:
The Registered Dietitian / Community Health Planner will function in close collaboration with
Physicians, Nurse Practitioners, and Allied Health Care Professionals within the Family Health Team
to ensure patients receive comprehensive, competent, and appropriate care.
As a member of the interdisciplinary team, the Registered Dietitian / Community Health Planner will
provide medical nutrition therapy through individual visits, team visits, and group settings. This
role will also support the planning, development, implementation, and evaluation of health
promotion initiatives that promote patient education, chronic disease prevention, healthy lifestyle
behaviours, and community wellness.
The role includes supporting the organization’s health promotion and communication efforts through
the development of educational materials, presentations, social media content, and other
public-facing resources. Experience with Canva and social media content development is required.
Accountability:
The Registered Dietitian / Community Health Planner will report to the Manager, Operations and
Culture and is also accountable to the College of Dietitians of Ontario for the standard and
quality of service provided within their clinical responsibilities.

Duties and Responsibilities:

  • Provide direct patient care including nutrition assessments, counselling, nutrition care plans,
    monitoring, evaluation, referrals, and follow-up.
  • Provide medical nutrition therapy in individual appointments, team visits, and group
    settings.
  • Be available for individual and team consultations, care planning, and problem-solving.
  • Develop and/or make accessible educational materials appropriate to the needs, literacy levels,
    and understanding of patients.
  • Develop and deliver programs related to identified nutrition and health promotion needs at times
    and locations appropriate for patients.
  • Support health promotion initiatives related to nutrition, chronic disease prevention, healthy
    eating, wellness, and other priority health topics.
  • Create engaging, accessible, and evidence-informed health education resources for patients,
    staff, and the broader community.
  • Develop presentation materials, posters, handouts, newsletters, social media content, and other
    communication materials using tools such as Canva and PowerPoint.
  • Collaborate with team members to support the redevelopment and ongoing maintenance of
    the
    organization’s social media presence across all platforms.
  • Ensure public-facing health promotion materials are accurate, inclusive, accessible, and aligned
    with best practices in health literacy.
  • Work with and refer to other members of the care team, including physicians, nurse
    practitioners, nurses, pharmacists, social workers, community partners, and others, to ensure
    patients receive comprehensive, competent, and appropriate health care.
  • Keep consistent and accurate records of patient encounters using established charting formats,
    including the electronic medical record.
  • Assist with data collection for quality improvement and health promotion initiatives.
  • Support the evaluation of health promotion activities and recommend improvements based on
    patient, staff, and community feedback.
  • Work within the full scope of practice set out by the College of Dietitians of Ontario.
  • Maintain and develop professional competence through ongoing professional development.
  • Participate in the established performance evaluation process and professional development goal
    setting.
  • Actively participate in staff, team, and committee meetings as appropriate.
  • May be required to supervise and mentor dietetic students.
  • Perform other related duties as assigned by the Manager, Operations and Culture.

Qualifications:

  • Bachelor’s degree in Food and Nutrition or equivalent degree with an accredited dietetic
    internship or equivalent supervised practical training required.
  • Member in good standing with the College of Dietitians of Ontario.
  • Current registration as a Certified Diabetes Educator is considered an asset.
  • Master’s degree in Public Health, Health Promotion, Community Health, Nutrition, or a related
    field is considered an asset.
  • Experience developing and delivering patient education, group programming, and/or health
    promotion initiatives.
  • Demonstrated experience using Canva or similar design platforms to create educational,
    promotional, and presentation materials.
  • Demonstrated experience creating, supporting, or managing content for organizational social
    media platforms.
  • Experience developing presentation materials using tools such as PowerPoint, Canva, or other
    related platforms.
  • Skilled in identifying nutrition problems, assessing nutritional status of patients, developing
    nutrition care plans, and evaluating and monitoring the effectiveness of nutrition interventions.
  • Demonstrated experience in individual and group counselling.
  • Previous experience managing chronic diseases through nutrition interventions.
  • Demonstrated computer literacy is required to facilitate the use of electronic medical
    records.
  • Strong written communication skills with the ability to translate health information into clear,
    engaging, and accessible language for patients and the public.
  • Demonstrated ability in problem-solving using sound judgment and critical thinking.
  • Strong organizational, interpersonal, and communication skills.
  • Ability to work independently and cooperatively in a busy multidisciplinary team.
  • Experience working in primary care, community health, public health, or a Family Health Team
    setting is considered an asset.
  • Knowledge of health literacy, population health, chronic disease prevention, and community
    engagement strategies is considered an asset.
    Conditions of Employment:
  • As a condition of employment, the successful candidate is required to submit proof of COVID-19

  • Contributes to the team’s work by participating in meetings and committees.
  • Due to the nature of the position, flexible hours are required, including evenings until 8:00
    p.m.
  • May be required to work out of either office location in Chatham or Tilbury.
  • Will be required to work onsite.

Salary Range: $73,736 to $83,426

About the Role
The Registered Dietitian / Community Health Planner will work under indirect supervision and play a
key role in providing one-on-one nutritional assessments and counselling, as well as leading group
education sessions focused on chronic disease prevention and management, including weight
management, diabetes, cardiovascular health, and healthy living. This role also includes supporting
community health planning, health promotion initiatives, program development, patient education
resources, and outreach activities that respond to the needs of patients and the broader community.
Working collaboratively within an interdisciplinary primary care team, the successful candidate
will support patients’ overall health goals while contributing to wellness, prevention, and
community engagement initiatives.

Qualifications

  • Current Certificate of Competence as a Registered Dietitian with the College of Dietitians of
    Ontario (or
  • equivalent).
  • Master’s degree in Public Health, Health Promotion, Community Health, Nutrition, or a related
    field is considered an asset.
  • Experience in a primary care, community health, or similar healthcare setting is an
    asset.
  • Experience with Accuro EMR would be an asset.
  • A clear vulnerable sector police check is required as a condition of employment.
  • Proof of COVID-19 vaccination is also required.
