Nurse Practitioner – 37.5 hours/week, Permanent Full-time

Overview
The Nurse Practitioner (NP) is an integral member of the Peterborough Family Health Team (PFHT),
working collaboratively with physicians, nurses, Interprofessional Healthcare Providers (IHPs), and
administrative staff to improve access to high-quality primary care across our community.

This is a permanent 1.0 FTE position working 37.5 hours per week. The NP will be based primarily at
the PFHT Connect Clinic, providing care to individuals in our community who do not currently have a
family physician or nurse practitioner. As a flexible organizational resource, the NP may also be
temporarily assigned to support other PFHT-affiliated Family Health Organizations (FHOs), programs,
or clinical services in response to changing patient, staffing, and organizational needs.

This role is well suited to an NP who enjoys variety, collaborative practice, and the opportunity
to work across different primary care environments while contributing to improved access to care
throughout the community.

The position reports to a Clinical Manager.

Responsibilities
The NP will practice to their full scope as outlined in the College of Nurses of Ontario Standards
of Practice:

• Complete comprehensive and focused health assessments, including health histories and physical
  examinations.
• Provide wellness care, health screening, health promotion, and disease prevention.
• Assess, diagnose, and treat episodic illnesses, minor illnesses and injuries, and stable chronic
  conditions using evidence-based guidelines.
• Prescribe medications and provide pharmacological and counselling interventions within scope of
  practice.
• Determine the need for, order, and interpret diagnostic investigations, including X-rays, ECGs,
  ultrasounds, MRIs, and CT scans.
• Develop and implement appropriate treatment, follow-up, referral, and care plans based on
  clinical findings and individual patient needs.
• Provide patient education and counselling to support informed decision-making and
  self-management.
• Collaborate with physicians, nurses, IHPs, administrative staff, and other members of the
  primary care team to support coordinated, high-quality care.
• Consult with physicians in accordance with applicable standards for physician
  consultation.
• Maintain complete, accurate, and timely electronic medical records and required clinical
  and statistical documentation.
• Provide clinical support across PFHT-affiliated FHOs, programs, and services based on identified
  organizational needs, including temporary coverage related to vacancies, leaves, increased patient
  demand, or other operational priorities.
• Adapt to different primary care environments, patient populations, clinical teams, and workflows
  while working collaboratively with teams at assigned locations.
• Participate in PFHT, FHO, team, and other meetings as required and contribute to quality
  improvement initiatives.
• Precept Primary Care NP students where appropriate and operationally feasible.

Hours of Work:
Hours of service are pre-established through the employment agreement and may require some evenings
and weekends. Scheduling and work location may vary based on patient care and organizational needs.

While the Connect Clinic will serve as the NP’s primary work location, the successful candidate
must be able to work at other PFHT-affiliated clinical locations within the Peterborough area as
required.

Any deviation from the established schedule shall not negatively impact the provision of service to
the patient population.

Qualifications

• Current RN registration with the College of Nurses of Ontario in the Extended Class.
• Successful completion of an approved Nurse Practitioner education program.
• Minimum 3 to 5 years of community health nursing experience, preferably within primary care
  and/or a rural setting.
• Demonstrated ability to work independently and collaboratively within interdisciplinary
  teams.
• Strong clinical assessment, decision-making, communication, and organizational skills.
• Demonstrated adaptability and ability to work effectively across different clinical environments
  and teams.
• Experience with electronic medical records and primary care clinical workflows is an
  asset.
