The Owen Sound Family Health Team (OSFHT) is seeking a 0.4 FTE Primary Care Clinical Pharmacist to join our team. The OSFHT is a team of allied health who are committed to improving and maintaining the health of the community. Based on local health and community needs, we provide access to innovative, high quality and comprehensive primary health care programs and services.
The Pharmacist is an active member of our multidisciplinary team who practices patient-centered and evidence-based care to achieve optimal drug therapy outcomes for our rostered patients. Our clinical pharmacist is the medication management expert of the team.
The Pharmacist will work within the full scope of practice set out by the Ontario College of Pharmacists (OCP) and will report to the Programs & Services Manager, and the Executive Director.
DUTIES & RESPONSIBLITIES
- Provide direct patient care in partnership with patients and other healthcare professionals
- Medication management by identifying drug related problems through medication reviews and patient interviews then following through to resolution.
- Being a resource for physicians, nurse practitioners and other allied health professionals in providing evidence based, timely and unbiased drug information
- Helping patients achieve their Smoking Cessation goals
- Provide medication reconciliation upon request from physicians, nurse practitioners and nurses
- Working with patients individually and in group settings, to educate patients and caregivers to help them better manage their health through medication.
- Maintain positive channels of communication & cooperation with the patients we serve, as well as other team members.
- Participate in the development of OSFHT programs and will work with the multidisciplinary team, patients, and community partners to provide integrated services to meet the health needs of individuals, families, groups, communities and populations.
- Provide feedback to management regarding needs or suggestions for facilitating optimum personal or professional (office) efficiency
- Comply with OSFHT Policies & Procedures
- Adherent to the Health and Safety Policies and Procedures, with regard to own safety and that of other people on premises
- Other duties as assigned.
QUALIFICATIONS & REQUIREMENTS
- Bachelor of Science in Pharmacy from a recognized university
- Current registration with the Ontario College of Pharmacists
- Additional training in pharmacotherapy preferably CAMH TEACH certification, CRE, &/or CDE would be an asset
- Two years of demonstrated experience in a primary care, hospital or community setting preferred
- Strong interest and experience in program development, implementation and evaluation
- Ability to work effectively and collaboratively within a multidisciplinary team and individually
- Proven efficiency working in a fast-paced environment
- Keen attention to detail and accuracy
- Commitment to best practices and quality patient care
- Able to demonstrate an understanding of PHIPA and confidentiality
- Excellent computer skills
- Experience with Microsoft Office is an asset
- Experience with an Electronic Medical Record System particularly TELUS PSS is an asset
Excellent work performance and attendance record are essential

Pay: $ 51.24 per hour
Expected hours: 15 per week
On-site parking
Paid time off
French not required
Schedule: 8-hour shift
Monday to Friday, no weekends
Work Location: In person – Owen Sound
