An exciting opportunity is now available for a New, Permanent Full-Time System Navigator to work in a collaborative model of care in Northumberland County as part of an interdisciplinary team. The successful candidate will assist clinicians and patients in navigating the healthcare system providing personalized support and ensuring high quality, patient-centered care so that patients achieve their healthcare goals. Duties might include, among others: understanding available community resources and how to access them; determining patient needs and where gaps in care lie; coordinating patient care plans and communicating with patients, caregivers, healthcare providers and other community services; facilitating education for patients, caregivers and healthcare providers on available resources locally and beyond; program evaluation and development to ensure efficiency in meeting the needs of our community. This position comes with paid vacation and an opportunity to enroll in the Healthcare of Ontario Pension Plan (HOOPP).

Qualifications, Skills and Experience:
- A degree in social work, nursing or health promotion (all levels to be considered), or equivalent
- Excellent interpersonal, communication, documentation and presentation skills
- Knowledge of community health and social services available locally and regionally
- Ability to work independently and as part of a collaborative team
- Efficiency in analyzing, problem-solving, organizing resources and priority setting
- Demonstrated ability to effectively mitigate and manage potential conflict
- Proficiency in the use of computers and electronic medical record systems
- Previous experience in a primary or other healthcare setting an asset
- Basic CPR certification an asset
