Couchiching FHT – Nurse Practitioners

The Couchiching Family Health Team is expanding, and we are looking for NP’s to join our team to
work towards our attachment strategy.


HOOPP: Effective April 25, 2026, the CFHT is a participating employer of the Healthcare

Position: Nurse Practitioner (Extended Class)

(1.0 FTE Permanent with Benefits)
Interim Reports To: Director of Clinical Services

Position Summary:
As a key member of the Couchiching Family Health Team (CFHT), and an integral part of an
interdisciplinary health care team, the Nurse Practitioner (NP) Extended Class (EC) uses advanced
practice clinical skills to provide direct clinical care, education, research, consultation, and
treatment to support the care needs of patients in the Couchiching region. This role may provide
care to both unattached and rostered patients in collaboration with Primary Care Practitioners. The
NP will establish relationships with Primary Care Practitioners (PCP’s), CFHT Allied Health
Practitioners, and community partners to provide best practice care to those without a PCP. The
Nurse Practitioner will apply an anti-oppressive lens in supporting diverse patients, including
those facing systemic barriers.

Roles and Responsibilities: Assessment:

  • Assess, monitor, provide intervention, education, and support to patients and their caregivers
    within the scope of the regulatory practice as delineated by the College of Nurses of Ontario
  • Complete health assessments, including health history and physical examination
  • Provide treatment and advanced clinical management as needed across the continuum of
    care
  • Utilize diagnostic reasoning and decision-making process to formulate a list of hypotheses,
    determine the need for, order, and interpret diagnostic testing, and formulation of a medical
    diagnosis within the specialty scope of practice and consistent with practice guidelines
  • Conduct consultations with clients/family to obtain relevant information and assess
    client/family learning and needs
  • Communicate significant clinical findings and facilitate ongoing communication of patient
    progress and plan of care to the health care team
  • Initiate/order, tailor and evaluate/discontinue selected medical and nursing therapy (including
    pharmacologic therapy) within the specialty scope of practice and consistent with practice
    guidelines and established medical directives to optimize the plan of care
  • Facilitates patient care by consulting with the patient/family and inter-professional patient
    care team to formulate, initiate, co-ordinate and evaluate a comprehensive and collaborative
    patient centred plan of care, ensuring effective and efficient use of resources
  • Promote continuity of care by initiating and co-ordinating patient discharge plans, warm
    transfers and integrated care methodology
  • Evaluate patient care outcomes and nursing practice issues to ensure the highest possible
    quality of care.
  • Prescribe medications according to scope of practice
  • Provide health education and health promotion information to individuals and groups
  • Link with community resources and share information to provide the best experience for the
    client
  • Ensure all notes and patient charts are updated in a timely manner and in accordance with
    College of Nurses and CFHT policies
  • Prepare quarterly reports and monitor daily statistical data to ensure statistics are reported
    to Ontario Health in a timely fashion
  • Participate in CFHT and regional committees as needed
  • Maintain and upgrade knowledge and skills
  • Maintain current knowledge of policies and procedures
  • Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection
    of Privacy Act
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures
    established by the CFHT
  • Other duties as required

Skills/Knowledge Requirements:

  • Demonstrated ability to work effectively as a member of an Interprofessional team, with
    excellent interpersonal and communication skills
  • Exceptional documentation skills and attention to detail
  • Knowledge and proficiency in current evidence-based methods and practices of primary care
    delivery, with an emphasis on health promotion and risk reduction
  • Demonstrated superior organizational, administrative, and problem-solving skills
  • A positive and professional attitude
  • Excellent time management skills with an ability to multi-task and effectively set
    priorities
  • Demonstrated working knowledge of community resources
  • Demonstrated knowledge of culturally responsive care, ability to work across difference and
    experience supporting equity-deserving patients
  • Demonstrated ability to work well autonomously
  • Adaptable to change
  • Knowledge in current, evidence-based methods and practice of primary care delivery
  • Excellent oral/written/interpersonal skills

Education/Specific Job Requirements:

  • Current registration in good standing with the College of Nurses of Ontario as an NP-EC
  • Mandatory Declaration of Professional Liability Protection upon hire, and annually
    thereafter
  • Registration with Registered Nurses Association of Ontario an asset
  • A Masters Degree in Nursing and completion of an Acute Care or Primary Care Nurse Practitioner
    Program required
  • Basic Cardiac Life Support (BCLS) Certification required
  • Demonstrated advanced clinical assessment, diagnostic and reasoning skills
  • Current Ontario Driver’s License and access to reliable vehicle
  • Able to work flexible hours including evenings and Saturdays when required
  • Police Record/Vulnerable Record Check
  • It is a condition of employment that you receive the necessary vaccinations to protect against
    infection of COVID-19, or any similar infectious diseases, which may be recommended by governmental
    and public health authorities from time to time

Experience:

  • Preference will be given to candidates with 2-3 years clinical experience as a Nurse
    Practitioner, preferably within primary care programs
  • Highly experienced with physical assessment as well as competence in emotional assessment to
    assist with developing an appropriate plan of care as per patient wishes
  • Experience with Practice Solutions, and virtual care such as PS Virtual
  • Experience with an Electronic Medical Record (EMR) and office applications.
