Clinical Pharmacist

0.8 FTE Part time in-clinic Position (32 hours per week)

The Belleville Nurse Practitioner-Led Clinic (BNPLC) is seeking a Part Time Pharmacist to join our
interdisciplinary health care team. Since 2010, the BNPLC has been a trusted leader in providing
accessible, individualized, fully responsive primary health care in the Quinte Region. With more
than 3,200 registered patients with a diverse range of needs, we make a difference every day.
Position Summary

As a key member of the Belleville Nurse Practitioner-Led Clinic (BNPLC), and an integral part of an
interdisciplinary health care team, the Pharmacist will provide pharmaceutical care to Belleville
Nurse Practitioner-Led Clinic (BNPLC) patients and serve as a resource to the team. This position
provides patient care and team collaboration on-site at the clinic and is not suitable for virtual
or remote work.

Duties and Responsibilities

• Provide primary health care to patients in accordance with standards set by governing body, and
  that is aligned with the overall strategic direction of the BNPLC
• Provide health promotion and education across the lifespan,
• Promote disease prevention and provide chronic disease management, primary mental health care
  support and primary palliative care support
• Enhance access to care
• Practice within a pharmacist’s legal scope of practice including prescribing smoking cessation
  aids, dose adjustments and prescription renewals;
• Collaborate and/or consult as appropriate, with members of an interdisciplinary team within the
  clinic and externally with the community of care
• Advocate, educate, lead, provide mentorship, and consult as needed
• Participate in committees or community groups where an identified need for advocacy or
  representation is required
• Design, implement and evaluate health care programs for the BNPLC in coordination with all
  members of the health care team; that address current gaps in service, target population or
  population health (e.g. STOP, OMSC (Ottawa Model for Smoking Cessation), Diabetes, INR)
• Works in collaboration with an Interprofessional team to safely and effectively provide
  appropriate primary care for patients.
• Perform patient medication reviews and reconciliation
• Identify and build relationships with community pharmacists who have expertise in particular
  drug/disease management areas (e.g. anticoagulation, cholesterol screening and monitoring,
  hypertension screening and monitoring, diabetes management, smoking cessation) and refer patients
  according to BNPLC protocols
• Answer drug related queries of BNPLC team members
• Educate and update BNPLC team members on the most current pharmacy or prescription guidelines
  and evidence-based research.
• Raise awareness and educate members of the BNPLC and the community on medication adherence
  issues and safe medication use.
• Monitor drug doses under clinical management plans.
• Foster an environment of safe medication practices within the community through systems analysis
  that includes improved interdisciplinary communication to encourage actions that support seamless
  care with the local health care community e.g. communicate changes in medications, errors, or
  patient concerns where appropriate.
• Plan, implement & evaluate health promotion, disease and accident prevention strategies and
  programs within the community
• Coordinate care or services for individuals or families
• Provide culturally sensitive care
• Record pharmaceutical assessments and interventions
• Submit statistics as required by Ontario Health and/or BNPLC
• Participate in training sessions as required by the BNPLC
• Participate in regular team meetings, and other meetings as requested
• Participates in the development, planning and evaluation of treatment, education, counselling
  and health promotion/disease prevention activities of the BNPLC.
• Facilitates group educational sessions and activities using the principles of child, youth,
  adult or older adult learning.
• Assists in the maintenance of a clean, orderly work environment
• Maintain confidentiality in accordance with the Personal Health Information Protection Act
  (PHIPA, 2004) and all other relevant legislation.
• Work in compliance with Health and Safety laws adhering to safe work practices and procedures
  established by the BNPLC.
• Other duties as required
  

Skills/Knowledge Requirements:

• Demonstrated ability to work effectively independently and as a member of an Interprofessional
  team, with excellent interpersonal and communication skills
• Exceptional documentation skills and attention to detail
• A positive and professional attitude
• Excellent time management skills with an ability to multi-task and effectively set
  priorities
• Knowledge in current, evidence-based methods and practice of primary care delivery
• Excellent oral/written/interpersonal skills
• Proficient in use of internet, Microsoft software, drug database programs
• Experience with an Electronic Medical Record, e.g., Telus EMR
• Experience in program design, delivery and evaluation an asset

Education/Specific Job Requirements:

• Baccalaureate Degree in Pharmacy
• Must possess a minimum of 2-5 years of experience in a primary care or similar practice
  setting;
• Licensed and in good standing with the Ontario College of Pharmacists (Part A);
• Current registration with Ontario Pharmacists Association and evidence of Professional Liability
  Insurance coverage
• Additional training or credentialing in pharmacotherapy (e.g. Certified Geriatric Pharmacist,
  Certified Diabetes Educator, Certified Asthma educator, etc.) is considered an asset.
• Certification in CPR
• Vaccinated against Covid-19 (minimum of two doses) or has valid medical exemption
• Able to work flexible hours including evenings
• Current Police Vulnerable Sector Check (less than 6 months old)

Physical Demands:
• The position requires minimal physical effort, and the workday comprises roughly equal amounts of
standing and sitting with frequent position changes.

Hourly rate range: $48.03 – $48.75 32 hours per week
