The Scarborough Academic Family Health Team (SAFHT) is governed by a Mixed Board (Physicians + Community Members). It is a multi-site Family Health Team located within Eastern Toronto using a collaborative model to provide comprehensive primary care for rostered and unrostered patients of its affiliated physicians. In the evolving Ontario Health Team environment, the SAFHT is also aligning with unaffiliated physicians to help achieve positive outcomes for all patients referred to its programs in primary care.

The Social Worker may be required to travel to various sites. We are seeking 1 permanent, full-time Social Worker to join our dynamic team of mental health professionals.

In collaboration with an interdisciplinary team, the Social Worker:
Assesses psychosocial needs of clients to identify factors which may affect the client’s ability to adjust and/or cope with their current situation.
Provides short-term, goal-oriented therapy to clients and family members where indicated.
Provides group education and group counselling where warranted.
Problem solving associated with high-risk indicators endangering clients’ mental and physical health.
Consults with physicians and other allied health professionals in SAFHT where warranted.
Coordinates delivery of care through appropriate communication with members of the client’s team within the circle of care.
Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required.
Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measurement.
Adheres to the code of ethics, in maintaining standards of practice for the profession as defined by the Ontario College of Social Workers and Social Service Workers.
Ensures that confidentiality of client information is maintained at all times.
Prepares statistical records of client care and related activities as required by SAFHT, the Ministry of Health, and other agencies.
In collaboration with the client, identifies the need for and assists in researching, planning and arranging community resources as required.
Identifies opportunities to further develop programs aimed at prevention, health promotion, and chronic disease management.
Advocates on behalf of clients and client groups where gaps in service have been identified.

ACCOUNTABILITY:
The Social Worker reports to the Executive Director of the Team or the delegate in case the Executive Director is unavailable. The Social Worker practices within the scope and according to the standards of:
Ontario College of Social Workers and Social Service Workers “Practice Standards” (www.ocswssw.org)
Regulated Health Professions Act (1991)

QUALIFICATIONS:
Master of Social Work Degree from a recognized university.
Current membership with the Ontario College of Social Workers and Social Service Workers.
Minimum of 3 years post-graduate clinical experience, preferably in a community and/or primary health care setting.
Proficiency in providing trauma-informed care and applying a range of evidence-based practices such as Cognitive Behavioural Therapy (CBT), Dialectical Behaviour Therapy (DBT), Solution Focused Brief Therapy (SFBT) is required.
Must possess strong knowledge in assessing and intervening with a broad range of psychosocial issues, including: depression, anxiety, trauma, relationship, life transitions, and substance abuse/concurrent disorders.
Experience working with multiple client populations across the lifespan.
Ability to engage clients quickly and encourage active participation in personal problem resolution.
Ability to work effectively with vulnerable and marginalized persons.
Self-motivated and demonstrated ability to manage a busy and varied caseload.
Proven flexibility and ability to adapt positively to change.
Excellent organizational, time management, interpersonal/communication skills.
Proficiency in the use of computer software, such as Microsoft Office and working familiarity with Electronic Medical Records (EMR) is preferred.

OTHER RESPONSIBILITIES:
Accepts client referrals from SAFHT affiliated physicians and unaffiliated physicians, as appropriate, and maintains an organized appointment schedule.
Shares results of assessments and interventions with referring physicians.
Triages referrals and manages appointment schedules, with the assistance of administrative staff.
Works in collaboration with interdisciplinary team and clients and families to safely and effectively plan, implement, coordinate and evaluate appropriate care to clients.
Works within a common practice framework with standards for assessments, communications, timeliness, charting using the EMR and best practice therapy modalities.
Implements various goal oriented, time-limited intervention plans, including education in collaboration with client and/or family members and inter-disciplinary team.
Assists in the planning, implementation and evaluation of health promotion and illness prevention programs and services.
Maintains relations with networks, related professional organizations and other related providers.
Participates in team building, program development and improvement activities.
Acts as a resource to team members and assists with staff education on Social Work services.
Participates in quality improvement initiatives.
Maintains and develops professional competence through ongoing professional development.
Provides timely and informative reports as directed, including statistical reports.
Other tasks as operationally required.
