Responsibilities:
Provide direct primary care to patients of all ages
Conduct nursing assessments, triage, treatments, and health education in accordance with CNO standards
Maintain accurate and timely documentation; participate in chart reviews, case conferences, and referrals
Administer immunizations and report per Ontario Immunization Guidelines
Stay current with nursing best practices and pursue ongoing professional development
Deliver direct primary care to patients of all ages, including wellness checks, well-baby visits, and injections
Support chronic disease management, preventative screening, and health promotion
Collaborate with physicians, nurse practitioners, and allied health professionals
Participate in program development and quality improvement
Opportunity to mentor and support interprofessional learners and students
Deliver care in clinic, community, and home settings
Lead infection control initiatives and report communicable diseases
Perform other duties as required to support evolving team needs

Qualifications
Diploma or Bachelor’s Degree in Nursing
Current registration in good standing with the College of Nurses of Ontario
Membership with the Registered Nurses Association of Ontario
Experience in primary care nursing (asset)
Proficiency with Electronic Medical Records (Telus PS Suite preferred) (asset)
Bilingualism (asset)
