About Us

The Athens District Family Health Team is located in beautiful Eastern Ontario, between the historic Rideau Waterway and the world-famous Thousand Islands. We are a small, supportive, and genuinely close-knit team that values collaboration, respect, and looking out for one another.

Our multidisciplinary team includes physicians, nurse practitioners, nurses, a health educator, and dedicated administrative staff. We believe that a positive workplace culture is just as important as excellent patient care.

The Opportunity

We are seeking a full-time Nurse Practitioner to provide comprehensive primary health care services to our roster of approximately 4,100 patients. Reporting to the Lead Physician and Executive Director, you will work in a highly collaborative environment where team members share knowledge, support one another, and celebrate each other’s successes.

What You’ll Do

  • Provide a full scope of primary health care services, including preventive care, health promotion, and timely acute care.
  • Deliver patient-centred care in collaboration with physicians, nurses, allied health professionals, and administrative team members.
  • Support health promotion programming and contribute to improved access to care for our community.
  • Collaborate with community partners providing laboratory, diabetes, lung health, eye health, and mental health services.
  • Help maintain a welcoming, respectful, and encouraging workplace where ideas are heard and contributions are valued.

What We Offer

  • Salary range of CA$120,000-125,000 per year
