NURSE PRACTITIONER

THE RIDEAU FAMILY HEALTH TEAM
The Rideau Family Health Team is a well-established, proud, multicultural family health team
located in Ottawa, Ontario. Our mission is to provide high-quality, comprehensive, and personalized
primary care to our patients and their families. Our team consists of a multi-disciplinary team of
allied health professionals including nurse practitioners, registered nurses, a dietitian, a
pharmacist, psychotherapists, social workers, and administrative support staff. We work alongside
our partnering primary care clinics, primarily in the southeast region of Ottawa.

POSITION SUMMARY
The Nurse Practitioner (NP) plays a critical role in delivering comprehensive primary care within
the Rideau Family Health Team. This position requires strong clinical expertise, sound judgment,
and the ability to work both independently and collaboratively within an interprofessional team.
The successful candidate will practice to the full scope of the RN (EC) class, providing
assessment, diagnosis, treatment, and ongoing management for urgent care and rostered patients.

• 1.0 FTE Nurse Practitioner, Urgent Care & Programing
• 2.0 FTE Nurse Practitioner, Most Responsible Provider (MRP)

ROLES AND RESPONSIBILITIES
Clinical Care

• Perform comprehensive health assessments, including history taking and physical
  examinations
• Diagnose and manage acute and chronic conditions using evidence-based clinical
  decision-making
• Order, interpret, and act on diagnostic tests
• Prescribe medications and initiate, adjust, or discontinue treatments within scope of
  practice
• Provide health promotion, disease prevention, and patient education

Care Coordination & Collaboration

• Develop and implement patient-centred care plans in collaboration with patients, families, and
  the interprofessional team
• Communicate significant clinical findings and care plans effectively with team members
• Facilitate continuity of care through appropriate follow-up, referrals, discharge planning, and
  community linkages

Documentation & Quality

• Maintain accurate, timely clinical documentation in accordance with CNO standards and
  organizational policies
• Monitor patient outcomes and contribute to continuous quality improvement initiatives
• Track and report required clinical data and statistics for organizational reporting to Ontario
  Health

Professional Practice

• Participate in team, organizational, and regional committees, as appropriate
• Maintain current knowledge of best practices, clinical guidelines, and organizational
  policies
• Adhere to privacy legislation, health and safety standards, and professional accountability
  requirements
• Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection
  of Privacy Act
• Other duties, as required

SKILLS AND KNOWLEDGE

• Advanced clinical assessment, diagnostic reasoning, and patient management skills
• Strong interpersonal, communication, and collaboration skills
• Ability to work both independently and as part of an interprofessional team
• Excellent organizational, time management, and prioritization abilities
• Proficiency in evidence-based primary care, health promotion, and risk reduction
• Strong documentation skills and attention to detail
• Experience working with diverse populations and a commitment to culturally responsive
  care
• Knowledge of community resources and care navigation
• Adaptability in a dynamic clinical environment

EDUCATION AND TRAINING

• Current registration in good standing with the College of Nurses of Ontario as an NP-EC, required
• Master’s degree in nursing and completion of an approved Nurse Practitioner program, required
• Current Basic Cardiac Life Support (BCLS) certification, required
• Level C CPR with Automated External Defibrillator training

ADDITIONAL REQUIREMENTS

• 1–3 years of recent clinical experience (primary care experience preferred)
• Experience with EMRs (Oscar Pro is an asset) and Microsoft Office
• Valid driver’s license and ability to travel between sites
• Ability to work flexible hours, including evenings and occasional weekends
• Fluency in English (written and spoken)
• Police Record/Vulnerable Sector Check
• Professional liability protection (required upon hire and maintained annually)
• Membership with RNAO and/or NPAO considered an asset
• Must be legally entitled to work in Canada
• Must be fully vaccinated against COVID-19 (2 doses). Only vaccines that Health Canada have
  approved and determined to be safe and effective for use in Canada & Ontario, are considered
  acceptable
  

WORKING CONDITIONS

• Full-time, permanent (37.5 hours/week)
• Primary locations:
  o 4–3625 Rivergate Way, Ottawa
  o 10–3771 Spratt Road, Ottawa
• Some evening and weekend work may be required

COMPENSATION

• Salary: $125,000 to $130,000 annually
• Benefits: Extended health and dental coverage
• Professional Development: Paid continuing medical education (CME) and professional development
  days (per organizational policies).
