Call for Members – Queen’s Family Health Team Advisory Board (Volunteer Position)

Help Shape the Future of Primary Care in the Queen’s Family Health Team Community

We are seeking passionate and engaged community members to join our Queen’s Family Health Team
Advisory Board. This is an exciting opportunity to provide advice, share perspectives, deepen your
sense of social commitment, and help improve primary care services for patients, families, and the
broader community.

About the Queen’s Family Health Team
As an academic teaching clinic affiliated with Queen’s University, we offer a unique environment
where compassionate care meets hands-on learning. Our team includes family physicians, resident
doctors, a nurse practitioner, nurses, social workers, a dietitian, a pharmacist and more — all
working together to support the patients’ health and well-being.

About the Advisory Board
The QFHT Advisory Board serves as a forum for collaboration between healthcare providers, patients,
and community members. Board members provide valuable input on programs, services, policies, and
initiatives that support accessible, equitable, high-
quality, patient-centered primary care.

We Are Looking For Individuals Who:

• Have an interest in leadership.
• Wish to contribute to the improvement of healthcare services in their community.
• Bring diverse perspectives, experiences, and backgrounds including lived experience as a
  patient.
• Are committed to respectful collaboration and constructive dialogue.
• Can attend regular meetings and participate actively in discussions.
• Are willing to represent the needs and interests of patients, families, and community
  members.
  
  We welcome applications from:
  
• Community members with a range of professional backgrounds (e.g., may include but not limited to
  program development, human resources management, financial
  management/auditing services, resource development, advertising, risk management, quality improvement, performance measurements, functional planning, knowledge of the healthcare system, communications, community relations, government relations, change management, and technological expertise (including AI expertise.)
• Individuals and/or their caregivers with experience navigating the healthcare system
• Members of the Patient and Family Advisory Committee at Queen’s Family Health Team
• Representatives from community organizations
• People from diverse cultural, linguistic, and socioeconomic backgrounds
  
  Responsibilities
• Advisory Board members will:
• Attend regular board meetings (6 to 7 times per year) (Typically there are no meetings scheduled
  for July and August.)
• Review meeting materials in advance.
• Provide feedback and recommendations on primary care initiatives.
• Help identify opportunities to improve patient and community experiences.
• Support a culture of equity, inclusion, and patient-centered care.
  

Commitment
Term: Three years (renewable for up to two additional terms)
Meeting Frequency: 6 to 7 meetings per year with a break over the summer months
Meeting Format: Hybrid format although in-person attendance whenever possible is encouraged.
Estimated Time Commitment: 2.5 hours per meeting
