Position: Nurse Practitioner (NP)
Status: Full-Time – 1 Vacancy
Compensation: $120,000 to 127,000 annually (plus % in lieu of benefits; salary commensurate with experience)

Position Summary

The Nurse Practitioner (NP) provides comprehensive primary care within full scope of practice, working both independently and collaboratively with patients and the interdisciplinary team to support the Vision and values of WMCFHT. The role focuses on patient-centered care, chronic disease management, health promotion, and prevention, while ensuring quality and safety. The NP demonstrates clinical expertise in assessment, diagnosis, ordering and interpreting tests, prescribing medications, and performing authorized procedures.

Key Responsibilities

  • Provide comprehensive primary health care, including health assessments, diagnosis, treatment, and health promotion across the lifespan.
  • Develop, implement, and evaluate health education, counseling, and disease prevention programs.
  • Order and interpret diagnostic tests (lab, imaging, ECG, ultrasound) and manage follow-up care.
  • Prescribe medications and therapies in accordance with NP scope and regulations.
  • Manage chronic and stable illnesses using evidence-based approaches.
  • Collaborate with physicians and other health professionals for referrals, consultations, and coordinated care.
  • Maintain accurate and timely medical documentation using the Electronic Medical Record (EMR) system.
  • Participate in program development, quality improvement initiatives, and interdisciplinary committees.
  • Supervise and mentor Nurse Practitioner students.
  • Provide patient care during regular and extended hours on a scheduled and urgent basis.

Qualifications & Experience

  • Baccalaureate of Science in Nursing (required), Master of Science in Nursing
  • Current Certificate of Registration with the College of Nurses of Ontario (RN–EC class)
  • Membership with RNAO and NPAO
  • Minimum two years clinical experience in family medicine or a related ambulatory setting (new graduates considered)
  • Strong leadership, assessment, diagnostic, and clinical decision-making skills
  • Knowledge of evidence-based primary care, health promotion, and chronic disease management
  • Demonstrated ability to work independently and as part of an interdisciplinary team
  • Excellent communication, organizational, and interpersonal skills
  • Proficiency in Microsoft Office and Electronic Medical Records (EMR) systems
  • Commitment to continuous learning and professional development
  • Flexible availability (some evening and weekend work may be required)
