Nurse Practitioner:
0.8 FTE (28 hours/week)

i. KFHT Main Site: 1201 Queen Street, Lower Level, Kincardine, ON
ii. KFHT Satellite Site: 18 Tain Street, Ripley, ON
The Kincardine Family Health Team (KFHT) is seeking a Nurse Practitioner to join our team. The KFHT
is a group of allied healthcare professionals with a mission to provide comprehensive team-based
primary patient care that is responsive to the needs of the community. As a Nurse Practitioner with
the KFHT, you will have a passion for providing primary care in a rural community including primary
care, emergency room diversion, chronic disease management, health promotion, and working with a
supportive and collaborative interdisciplinary team.
The Nurse Practitioner will work within the full scope of practice set out by the College of Nurses
of Ontario for Registered Nurses in Extended Class and report to the Executive Director, Clinical
Lead, and Consulting Physicians.
Roles and Responsibilities
- Participate in Primary Care attachment program, assisting with program design and
implementation. - Participate in Emergency Room diversion through our Same-Day Access clinic with a focus on acute
episodic care. - Potential Involvement with several programs delivered by the KFHT, including, but not limited to
Cardiac Rehabilitation and Memory Clinic. - Complete health assessments including health history and physical examinations, ordering
diagnostic and lab tests, formulating and communicating medical diagnoses and disorders, referring,
and prescribing medications in accordance with the NP scope of practice. - Provide primary, secondary, and tertiary care through screening, monitoring, and
pharmacological/counseling interventions. - Determine the need for, and the implementation of health promotion/prevention strategies for
families and individuals. - Consult with physicians in accordance with the standards for physician consultation.
- Work closely with fellow team members, Clinical Lead, and the Executive Director to monitor and
evaluate services and programs for quarterly NP reports required by Ontario Health. - Provide mentorship and additional skills training for co-workers and students.
- Participate in the development of KFHT programs and will work with the multidisciplinary team,
patients and community partners to provide integrated services to meet the health needs of
individuals, families, groups, communities and populations
Education / Job Specific Requirements
- Current registration with the College of Nurses of Ontario
- Nurse Practitioner Extended Class, Primary Care certificate
- RNAO / NPAO membership preferred
- Three years demonstrated experience in a primary health care (family medicine) or community
setting preferred. - Strong interest and experience in program development, implementation, and evaluation
- Able to function as a team player working collaboratively with other KFHT staff and community
partners - Sound organizational, teaching and leadership skills
- Proficient in the use of Electronic Medical Record and virtual communication platforms
- Commitment to working from a community-based and health equity perspective

- Proven efficient and effective patient care and ability to leverage existing resources when

Compensation: $63.59/hour – $68.94/hour
