Position: Registered Social Service Worker (1.0 FTE Position, Permanent with Benefits) Reports To:
Program Manager

Position Summary:
The Registered Social Service Worker (RSSW) supports individuals in navigating health and social
care systems by identifying needs, addressing barriers, and facilitating timely access to appropriate
services. The RSSW works with individuals experiencing social, financial, and health-related
barriers, providing short-term, solution-focused support to reduce obstacles and improve access to
care and community resources.


This position will establish and maintain a network within the community, creating strong linkages
between healthcare and social service agencies, while assisting in the development of coordinated
care plans for individuals. Working as part of an interprofessional team, the RSSW contributes to
coordinated, person-centered care and improved overall well-being.

Roles and Responsibilities:

  • Support the development of coordinated care plans for individuals receiving support from the
    System Navigation team
  • Conduct assessments to identify individual goals, needs, and barriers, and provide system
    navigation support
  • Facilitate connections and referrals to appropriate community services and resources (e.g.,
    income supports, housing, mental health services)
  • Advocate on behalf of individuals to address social and systemic barriers to care
  • Build and maintain strong community partnerships, including with organizations such as ODSP,
    Ontario Works, and community housing providers
  • Identify service gaps, duplication, and emerging needs within the community and communicate
    findings to the team
  • Complete required forms to support patient goals
  • Sit on core working groups as needed
  • Provide culturally sensitive care
  • Conduct meetings with individuals in home, community, or office settings, based on need and
    accessibility and adhering to the Home Visit Policy Guidelines
  • Conduct in-services and workshops for team members, other health professionals and community
    partners as appropriate

Additional Roles and Responsibilities:

  • Maintain current knowledge of policies and procedures
  • Manage personal health and social information in accordance with privacy laws and ethical
    standards.
  • Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection
    of Privacy Act.
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures
    established by the CFHT
  • Other duties as required
    Skills/Knowledge Requirements:
  • Strong understanding of healthcare and social service systems, including community-based
    programs and eligibility pathways
  • Knowledge of social determinants of health and their impact on access, equity, and
    outcomes
  • Familiarity with mental health, addictions, housing, income supports, and community
    resources
  • Understanding of trauma-informed, culturally safe, and person-centered care approaches
  • Knowledge of privacy legislation and documentation standards (e.g., PHIPA)
  • Awareness of local and regional service networks and system navigation pathways
  • Excellent communication and organizational skills
  • Demonstrated ability to work independently and in a team environment
  • Demonstrated knowledge of culturally responsive care, ability to work across difference and
    experience, supporting equity-deserving clients

Education/Specific Job Requirements:

  • Degree or Diploma in Social Service Work, and registration with the College of Social Workers
    and/or Registered Social Service Workers of Ontario
  • Certification in CPR (BLS-C)
  • Current Ontario Driver’s License
  • Able to work flexible hours including evenings if required
  • Ability to travel within the community
  • Police Record Check
  • It is a condition of employment that you receive the necessary vaccinations to protect against
    infection of COVID-19, or any similar infectious diseases, which may be recommended by governmental
    and public health authorities from time to time

Experience:

  • 2 to 3 years related or applicable experience in a community, hospital or primary health care
    setting
  • Demonstrated experience supporting individuals with complex social, financial, and
    health-related barriers
  • Experience working within or alongside healthcare systems, community agencies, or
    interdisciplinary teams
  • Proven ability to navigate and connect individuals to community resources, including housing,
    income supports, mental health, and addictions services
  • Experience working with ODSP/OW, housing, and providing intensive case management in a home
    setting is required
  • Experience with Practice Solutions an asset
