Instructions for completing your membership application:

  1. Download the AFHTO membership application form.
  2. Please insert your FHT budget information in the appropriate category.  The corresponding Membership Fee is your Interim Fee for the 2012-2013 membership year.
  3. Review and update your FHT information in the Membership Application (on pg. 2).
    1. Complete the form online and e-mail to info@afhto.ca.
  4. Print the completed invoice and application form, mail to AFHTO with cheque payable to:
