Instructions for completing your membership application:
- Download the AFHTO membership application form.
- Please insert your FHT budget information in the appropriate category. The corresponding Membership Fee is your Interim Fee for the 2012-2013 membership year.
- Review and update your FHT information in the Membership Application (on pg. 2).
- Complete the form online and e-mail to info@afhto.ca.
- Print the completed invoice and application form, mail to AFHTO with cheque payable to:
