Learning Objectives

Participants will leave the session with:

  • A clear understanding of how OCEAN functions from within EMR, or internet browser, to improve efficiencies of referral process for patients and providers
  • An understanding of the value of OCEAN messaging function – linking providers and keeping patients informed of action on their referral
  • An awareness of the available analytic data and how such data can help primary care providers make decisions about patient care
  • Information about their regional deployment team to enable further conversations and adoption within their primary care practice  

Summary/Abstract

OCEAN eReferral provides a foundation on which communication with providers can be enhanced within a secure digital realm from within their integrated EMR.  It provides access to real time updates around high-volume services and enables patients the opportunity to be more informed participants in their care. This includes confirmation that their referral was sent, as well as appointment confirmation and details (e.g. where to go, what to bring, what to expect). Communication between the sender and receiver is enhanced as referral receivers through status updates and a secure messaging function. During the session, participants will see how Ocean eReferral can easily be incorporated within their primary care workflow to send, track and manage electronic referrals using an integrated EMR (Telus PS, Accuro, and Oscar) or an internet browser. The Ocean Health Map provides primary care members with a directory of specialist and specialty services searchable by service, patient address, patient location preference, and real-time wait times. Ocean eReferral is currently being deployed at  in the WWLHIN, SELHIN, Champlain LHIN, NELHIN, and ESCLHIN through the System Coordinated Access (SCA) Program. Teams in each of these five regions provide guidance and training to support the adoption of this digital tool into practice. Participants will leave with information about who to contact in their community for support with implementing Ocean eReferral into their primary care team.
