This 90-minute webinar will:

  • Provide an overview of the toolkit

Explore examples of Community-Based Rehab Programs that have implemented EQ-5D-5L and PREMs. Hear about their successes, challenges and opportunities and how they used the data at a patient-provider level and/or organizational level

Presenters:

Dr. Fatima Al Sayah, PhD

Fatima is a renowned health outcomes researcher at the University of Alberta's School of Public Health, and is an expert in measurement of patient-reported outcomes (PROs) and quality of life, and their application in clinical and cost-effectiveness research, population health assessment, and clinical practice. Fatima serves as the research manager of the Alberta PROMs and EQ-5D Research and Support Unit (APERSU). She is a principal and PROMs lead at the Centre for Clinical, Health Economics and Outcomes Research (CCHO) in Beirut and Dubai. Fatima is a valued member of the EuroQol Group and serves on multiple committees and working groups. With over 75 publications, Fatima serves as an Associate Editor of the Health and Quality of Life Outcomes journal, and is a regular reviewer for other leading journals including Quality of Life Research and Value in Health.

Jeanne Bonnell

Jeanne is a Director at Ontario Health atHome, Champlain. Collaborating with regional partners including people with lived experience, and drawing on her background as an occupational therapist, Jeanne established the Champlain Community Stroke Rehabilitation Program as a small pilot project in 2016. Since then, the program model expanded to most of the Champlain Region and now provides specialized interdisciplinary rehabilitation to stroke survivors following hospital discharge. She is currently Chair of the Champlain Regional Stroke Network Steering Committee and a member of the CorHealth Ontario Stroke Leaderships Council. Jeanne’s other portfolios include Enhanced In-Home Remote Care Monitoring Program, Rapid Response Nursing, Allied Health, Fall Prevention, and the Senior Fitness Exercise Program, all with a focus on integrating a team-based rehabilitation perspective/model into broader health system change.

Gabrielle Sadler
Gabrielle is a project manager with the Rehabilitative Care Alliance. Her experience as a physiotherapist, health care administrator and LEAN Six Sigma Black Belt spans both hospital and community settings, including transdisciplinary home programming, acute and critical care, inpatient rehabilitative care, stroke care, geriatrics and palliative care. Gabrielle leads provincial initiatives that support patient-centred development and implementation of rehabilitative care best practices across the continuum of care. 

 

The panel discussion will be moderated by Emily Stevenson. 
Emily is the Chair of the RCA Community-Based Rehab Task Group and Director of Community Rehabilitation at CommunitiCare Health.
