Objectives
Registration is now open for Health Innovation Group’s evidence and team-based approach to renewing primary healthcare across Canada. Interactive education opportunities are now open for all primary care providers and interprofessional teams for all practice profiles. These virtual programs start in the Fall 2023 and include early registration and group discounts (improve@healthinnovationgroup.ca).
1. National Patient’s Medical Home Expedition Program
When: Starting November 2023: 12 sessions, 4.5 hrs each
Join other teams in an immersive learning environment from across the country to explore high impact changes for your practice, test and implement them and share your experiences with a network of peers. Join us and register here
2. International Practice Facilitation Program for Patient’s Medical Home
When: Starting Fall 2023: 8 virtual sessions, 3.5 hours each
Join other individuals for an interactive training program to build the skills and knowledge for effective practice facilitation. Participants will learn to support primary healthcare practices in implementing evidence-based changes. This is being delivered in partnership with Health Innovation Action.
