Two Rivers Family Health Team provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients residing in the local community and surrounding area. The FHT employs an interprofessional team of health care providers who work in a collaborative model of care.

The FHT is currently seeking a Registered Nurse to join its growing team of health care professionals. Reporting to the Executive Director, the role will play an important role within the FHT operations. This position is a 0.6 FTE, three days a week. Position Profile
As a key member of the Two Rivers Family Health Team (TRFHT) and an integral part of an interprofessional health care team, the Registered Nurse (RN) will deliver nursing care in accordance with TRFHT policies and professional practice standards, and according to regulations set out by the College of Nurses of Ontario. You will perform routine nursing duties and ensure that the nursing needs of the TRFHT patients and physicians are met.

The Registered Nurse’s primary role is to work in partnership with TRFHT physicians and providers to perform patient care. According to The Nursing Act, the Nursing Scope of Practice Statement states that nursing is the promotion of health and the assessment of, and the provision of, care for, and the treatment of, health conditions by supportive, preventative, therapeutic, palliative and rehabilitative means to attain or maintain optimal function.

Roles and Responsibilities
Duties of the Registered Nurse will be divided into two categories – clinical duties and duties related to the health team.

RNs will be encouraged to operate within their full scope of practice. They will be encouraged to work cooperatively with all members of the health care team in the promotion of patient health and prevention of disease.

Clinical Duties
• Provide patient oriented, goal directed nursing care in accordance with the standards of care from the College of Nurses of Ontario
• Communicate ideas and concerns to the health care team and participate in problem solving
• Use knowledge, skill and judgment to triage incoming patient calls, assess patient’s health needs, discuss test results and respond to questions pertaining to health promotion and disease prevention
• Demonstrate an understanding of chronic disease management and patient self care strategies
• For patient appointments, where applicable, initiate the Electronic Medical Record (EMR) of the patient visit, purpose, history and health care needs. Record patient vitals and prepare patient and room for examination
• Perform certain tasks as delegated by the organization’s Medical Directives, within the areas of acute/chronic, prevention and health promotion care.
• Maintain accurate, concise and confidential EMR documentation to recall patients for health promotion and disease prevention follow up such as immunizations, pap smears, mammograms, FOB testing, bone density, smoking cessation, hypertension, breast feeding support and diabetes
• Continue professional educations and growth through the College of Nurses of Ontario Quality Assurance Program
• Attend relevant conferences, workshops and in services
• Participate in staff meetings and review current organizational operations
• Provide mutual support to other nursing staff within the FHT, colleagues and other staff members
• Maintain that all medical information provided is current and unbiased
• Function as a clinical leader/resource for administrative staff, MOAs and other disciplines

Interprofessional Collaboration
• Develop and hone collaborative community relationships to facilitate patient access to services upon physician/health care team request.
• Communicate ideas or concerns to the health care team. Participate in problem solving to enhance patient care.
• Assist in the training and development of new staff

Qualifications
• Must possess a License with the College of Nurses of Ontario as a Registered Nurse
• Two to three years of clinical experience
• Primary care experiences an asset
• Knowledge of and working experience with computer systems and applications
• Honed patient assessment and triage skills. Telephone triage an asset.
• Excellent verbal and written communication skills
• Ability to work co-operatively as a member of the FHT.
