Job Description

The Registered Dietitian (RD) will function in close collaboration with Physicians, Nurse Practitioners, and Allied Health Care Professionals within the Family Health Team to ensure patients receive comprehensive, competent, and appropriate care. As a member of the team, the RD will provide medical nutrition therapy in individual visits, team visits, and in group settings.

Accountability:

The RD will report to the Manager, Operations and Culture and is also accountable to the College of Dietitians of Ontario for the standard and quality of service provided in her/his clinical responsibilities. 

Duties and Responsibilities:

  • To provide direct patient care including assessments, counseling, nutrition care plans, monitoring, evaluating, referral and follow-up.
  • To be available for individual and team consultations and problem solving.
  • To develop and/or make accessible educational materials appropriate to the understanding of patients.
  • To develop and provide programs around identified nutrition issues at times and location appropriate for patients.
  • To work with and refer to other members of the care team (physicians, nurse practitioners, nurses, pharmacists, social workers, community partners and others) to ensure patients receive comprehensive, competent and appropriate health care.
  • To keep consistent and accurate records of patient encounters using established charting format (i.e. electronic medical record, or EMR).
  • To assist with data collection for quality improvement initiatives.
  • To work within the full scope of practice set out by the College of Dietitians of Ontario.
  • To maintain and develop professional competence through ongoing professional development.
  • Participate in established performance evaluation process and professional development goals.
  • To actively participate in staff, team and committee meetings as appropriate.
  • May be required to supervise and mentors dietetic students.
  • Collaborate with team members to support the redevelopment with social media presence across all platforms.
  • To perform other related duties as assigned by the Manager, Operations and Culture.

Qualifications: 

  • Bachelor's degree in Food and Nutrition or equivalent degree with an accredited dietetic internship or equivalent supervised practical training required, post-graduate education preferred. 
  • A member in good standing with the College of Dietitians of Ontario. 
  • Current registration as a Certified Diabetes Educator (an asset). 
  • Skilled in identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluating and monitoring the effectiveness of nutrition interventions. 
  • Demonstrated experience in individual and group counselling. 
  • Previous experience managing chronic diseases through nutrition interventions 
  • Demonstrated computer literacy is required to facilitate the use of electronic medical records.
  • Demonstrated ability in problem solving using sound judgment and critical thinking. 
  • Strong organizational, interpersonal and communication skills. 
  • Ability to work independently and cooperatively in a busy multidisciplinary team. 
  • Experience with social media, developing presentation materials (Canva, Power Point, etc.) would be an asset.

  • Due to the nature of the position, flexible hours are required including evenings until 8:00 p.m. You may also be required to work out of either of our office in Chatham or Tilbury.
  • Will be required to work onsite
