POSITION SUMMARY:

We are seeking a Program Manager to join our dynamic Family Health Team (FHT). This role is ideal for a motivated, relationship-driven leader who excels at working across disciplines to deliver high-quality, patient-centered care. The successful candidate will lead the development, coordination, and evaluation of clinical programs and services, working closely with our Interdisciplinary Health Professional (IHP) team, physicians, leadership, and community partners.

RESPONSIBITIES:

• Lead and collaborate with the IHP team to design and implement effective, data-driven health programs based on team insights and patient needs.
• Coordinate program operations, including IHP scheduling, task planning, and resource allocation, ensuring timely and efficient service delivery.
• Schedule and chair regular meetings with internal stakeholders including the Director of Operations, ED, physician leads, and IHPs to align on program goals and updates.
• Collect, analyze, and report on program data and outcome measures, presenting quarterly updates to leadership and physicians.
• Develop and manage the group program calendar and maintain a prioritized timeline of deliverables.
• Lead the development and implementation of the annual Quality Improvement Plan in partnership with the QIDSS, tracking progress and reporting outcomes.
• Monitor referrals and patient access to programs, ensuring equitable, timely, and coordinated care.
• Foster a collaborative, supportive environment by mentoring and assisting team members in program development and implementation.
• Liaise with external health service providers to promote community partnerships and integrated care delivery.
• Support continuous improvement by identifying operational challenges and contributing to solutions in risk management, patient relations, and clinical effectiveness.

SKILLS & COMPETENCIES:

• Proven leadership experience with the ability to motivate teams and drive change in a collaborative care environment.
• Strong verbal and written communication skills, with the ability to engage a variety of stakeholders inside and outside the organization.
• Excellent organizational, time management, and prioritization skills, with the ability to manage multiple initiatives simultaneously.
• Demonstrated strength in problem-solving, decision-making, and analytical thinking.
• Proficient with Microsoft Office (Outlook, Word, Excel, PowerPoint) and experienced in the use of Electronic Medical Records (EMRs).
• Thorough understanding of the Personal Health Information Protection Act (PHIPA) and commitment to privacy best practices.
• Ability to work effectively in a rapidly changing, team-based environment, including participation in staff and committee meetings.
• Experience in program evaluation and quality improvement is considered an asset.
• Experience in community outreach or engagement is also an asset.

QUALIFICATIONS/EXPERIENCE:

• Post-secondary degree in Science, Health, or a related discipline, or in Business Administration with relevant experience supporting or leading Interdisciplinary teams.
• Minimum of 2 to 3 years of recent experience (within the last 12 months) in a program administrator/manager role or similar position within a related field.
• Prior experience in a primary care setting is an asset.
• Must successfully complete a Criminal Reference Check for the Vulnerable Sector.
