Job Description

The NSFHT is a small multi-disciplinary team located in Northwestern Ontario. We provide primary care to patients in the communities of Terrace Bay and Schreiber. We offer a friendly, collaborative, and professional environment at two sites – 15 mins apart.

Position Summary:
As an integral member of the North Shore Family Health Team, the Social Worker delivers a wide range of mental health services. These services may include management of mental health issues, counselling, education, navigation, case management and advocacy. The social worker will manly service individual adults and groups.
