Job description

The Kincardine Family Health Team (KFHT) is seeking a Registered Practical Nurse (RPN) to join our team.

The KFHT is a group of allied healthcare professionals with a mission to provide comprehensive team-based primary patient care that is responsive to the needs of the community. As an RPN with the KFHT, you will support primary care programs in rural community settings including unattached patient care and new patient attachment, emergency room diversion, chronic disease management, and health promotion. The successful candidate will collaborate with a supportive and collaborative interdisciplinary team to support our patients.

The RPN will work within their full scope of practice set out by the College of Nurses of Ontario and report to the Executive Director, Clinical Lead, and Nurse Practitioners.

Roles and Responsibilities
• Support Nurse Practitioners and Physicians in the Primary Care attachment program, assisting with program design and implementation.
• Support the KFHT Same Day Access Nurse Practitioner clinic with a focus on acute episodic care.
• Support other interdisciplinary programs delivered by the KFHT as needed.
• Assist in waitlist management, contacting patients, gathering medical records, and patient intake for primary care attachment program.
• Greeting patients upon arrival, register the patient and confirm demographic information on patient record.
• Provide patients with support and guidance as needed.
• Performs various clinical procedures such as immunizations, injections, taking blood pressure, suture and staple removal, ear syringing, dressing changes, etc.
• Maintains accurate patient electronic medical record (EMR).
• Contact patients to remind them of upcoming appointments and assist with system navigation (lab, diagnostic imaging, referrals, specialist appointments).
• Follow-up phone calls to patients as required or directed by Nurse Practitioners or Physicians.
• Monitors supplies and equipment inventory.
• Cleaning of exam rooms between patients, cleaning tools, operate autoclave.
• Assists with patient forms and referrals as required.
• Other duties as assigned.

Education / Job Specific Requirements
• Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
• Current Professional Liability Insurance coverage.
• Basic First Aid & CPR (enhanced to include AED)
• Experience in a primary health care (family medicine) or community setting preferred.
• Able to function as a team player working collaboratively with other KFHT staff and community partners.
• Proficient in the use of Electronic Medical Record and virtual communication platforms (such as Telus PS Suite)
• Commitment to working from a community-based and health equity perspective
Skills and Abilities
• Excellent communication, organizational, administrative, and problem-solving skills
• Ability to provide health system navigation, leveraging existing resources when available

Compensation: $27.67/hour - $30.00/hour
