Job Description

The Kincardine Family Health Team is seeking Psychotherapist (Social Worker, Registered Nurse, or Occupational Therapist) to join our team of interdisciplinary health care providers.

The Kincardine Family Health Team (KFHT) is a group of allied healthcare professionals with a mission to provide comprehensive patient care utilizing an interdisciplinary approach that is responsive to the needs of the community. As a Psychotherapist with the KFHT, you will have an interest in working in a Team-Based Primary Care model, promoting mental health through involvement with KFHT programs, providing brief counselling intervention (6-8 sessions), and system navigation.

Roles and Responsibilities:
- Work within an interdisciplinary Team to receive and triage referrals
- Maintain caseload from intake to discharge from service
- Provide counseling, support, and system navigation consultation for patients and their families, from teenagers to adults.
- Collaborate with members of the interprofessional primary care team (family physicians, nurse practitioners, nurses, dietitians, occupational therapists, kinesiologist, respiratory therapist, midwife) to deliver comprehensive care and service coordination
- Involvement with programs delivered by our team
- Collaborate with partner organizations as appropriate to address patient goals and promote smooth transitions of care.
- Maintain complete and accurate electronic medical records
- Actively participate in program planning, quality improvement initiatives, and meetings as required
- Maintain professional and technical knowledge

Education / Specific Job Requirements:
- Masters level degree in Social Work OR,
- Masters level degree in Occupational Therapy with completion of instructional, theoretical, and practical training in Psychotherapy, OR
- Bachelor of Science in Nursing with completion of instructional, theoretical, and practical training in Psychotherapy
- Registered with the Ontario College of Social Workers & Social Service Workers, College of Occupational Therapists of Ontario, or the College of Nurses of Ontario
- Evidence of professional liability insurance
- Vulnerable Sector Screen
- Membership with a professional association preferred

Experience:
- 2 years of experience in a health care setting preferred, with experience in general counselling and psychotherapy approaches, primary health care, community health care, and/or hospital setting
- Demonstrated ability to work independently and collaboratively in an interdisciplinary team.
- Strong clinical and case management skills related to establishing, setting, treating, and monitoring patients’ goals and facilitating transitions in care
- Ability to exercise sound clinical judgement and to function in an independent, self-directed manner
- Knowledge of community resources, services and supports programs and barriers individuals face in accessing same (e.g.: Financial, Housing, Social Support).
- Proficiency in the use of EMR software applications and virtual communication platforms
- Commitment to working from a community- based and health equity perspective
- Excellent communication, organizational, administrative, and interpersonal skills

Compensation:
- $38.59 / hour - $41.84 / hour
