The Hanover Family Health Team (HFHT) is seeking a collaborative Primary Care Nurse Practitioner to join our rural primary care practice on a permanent full-time (up to 30 hours per week) basis.

Hanover FHT is a group of interprofessional healthcare providers associated with Hanover Family Health Organization (FHO), a group of family doctors in Hanover and surrounding area with a combined roster of 10,000 patients.

The Nurse Practitioner (NP) will work to full scope of practice and provide comprehensive primary care to a panel of patients as well as lead a range of HFHT programs.
Main Responsibilities
Work collaboratively with physicians and inter-disciplinary team members to effectively manage patients experiencing a spectrum of medical issues from new life events to episodic illness to chronic disease by:
-demonstrating advanced knowledge, skill and judgment in providing care to patients that is consistent with CNO Practice Standards as well as competencies defined by the Canadian Nurses Association. This includes the authority to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures.
- demonstrating a high degree of autonomy and clinical skill to formulate clinical decisions, appropriately manage acute and chronic illness, and promote wellness.
Requirements
• Baccalaureate degree in Nursing. Masters preferred.
• Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligible to practice as a RN-EC.
• Current membership with RNAO and evidence of liability insurance.
• Basic CPR certification.
• Experience in a Community Health or Primary Care setting is preferable.
• Advanced health assessment and clinical skills combined with sound clinical judgement and diagnostic skills relevant to dealing with a wide variety of ages and health concerns.
• Demonstrated ability to communicate effectively with patients, families, and members of an interdisciplinary medical team.
• Knowledge and proficiency in current, evidence-based methods and practices for primary care delivery, with an emphasis on health promotion and risk reduction.
• Experience in patient education and chronic disease management.
• Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient.
• Excellent interpersonal, written and verbal communication skills
• Current Ontario Driver’s License and access to a vehicle for travel within the region
• Up-to-date vaccinations (or willing to get up-to-date).

Salary/Benefits
· Hourly rate: $57.88-$68.47

Education
Baccalaureate degree in Nursing. Masters preferred.
