Salary: $108,237.00 - $125,477.00 depending on experience
Hours of work: 40 hours/week, Mon to Fri, between the hours of 8am to 5pm.

Our Family Health Team is seeking a Nurse Practitioner (NP) to join our team of 8. The successful applicant will work closely with physicians in their primary care offices, located in Creemore and Collingwood. You will facilitate the continuity of primary health care by working collaboratively with our physicians, and our Interdisciplinary Health Professionals.

We work in a professional and collegial environment and use an up-to-date electronic medical records system (EMR). The Georgian Bay Family Health Team (GBFHT) provides a host of primary care services including Diabetes education, Lung Health, Healthy Aging, Nutrition counselling, Cardiovascular Rehabilitation, MH counselling and Telemedicine Services.
Key responsibilities:
• Use research and evidence to inform clinical care, education, and planning.
• As appropriate, lead or participate in quality improvement initiatives.
• Build strong collaborative relationships with key internal and external partners and stakeholders.
Qualifications:
• RN in the Extended Class with a Graduate degree and registration in good standing, with the College of Nurses of Ontario (CNO)
Experience/Skill:
• Minimum 3 years’ experience working in a community and/or primary care setting preferred.
• Membership in professional associations and/or interest groups an asset.
• Valid Ontario driver’s license and operational vehicle.
• Ability to adapt to change, generate new knowledge and apply it in different ways to formulate and problem solve within a context of complexity and uncertainty is essential.
• Models and promotes core ethical practice and reflects an optimistic and positive attitude.
• Demonstrated competency required in:
Clinical assessment, diagnosis, intervention, escalation, and evaluation.
Prescribing of pharmaceutical and non-pharmaceutical interventions.
Interpersonal engagement and team building.
Decision-making, critical thinking and conflict resolution.
Risk management and optimizing patient safety.
Managing competing demands and priorities within defined timelines.
Verbal and written communication, including effective use of computer programs like Microsoft Office, PS Suite experience an asset.
Working autonomously and being self-directed.
