The Dufferin Area Family Health Team (DAFHT) is currently recruiting for one (1) full-time Mental Health Counsellor to join an interagency, collaborative team at HART Hub Dufferin (HHD).
HART Hub Dufferin (HHD) is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use issues. By providing integrated, low-barrier support services, we aim to enhance the quality of life, physical health, and overall well-being of individuals in our community. We take a person-centered, equity-focused approach to ensure that clients have access to the care they need when they need it.
We are seeking a dynamic and compassionate Mental Health Counsellor to be an integral part of HHD collaborating with a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals. The Mental Health Counsellor will provide brief mental health case management support and deliver time-limited, client-centered interventions using evidence-based approaches to monitor these interventions while working closely with a variety of professionals.
Reporting Function:
The Mental Health Counsellor will receive direction from the HHD Manager for day-to-day operations and be accountable to the DAFHT Mental Health Lead. The Mental Health Counsellor will work with both internal and external partners and will interact at all levels within the organization.
Duties and Responsibilities:
• Engage with and advocate for clients experiencing psychosocial stressors impacting their mental health including disability, illness, housing issues, unemployment, poverty, and/or loneliness
• Provide collaborative case management to assess needs, navigate services, and support clients
• Work with clients to identify gaps in strengths and resources, develop goals, objectives, and provide concrete support to address needs
• Refer clients to appropriate health and social services within the HHD
• Be a resource and support for clients applying for financial assistance programs, such as ODSP, Ontario Works, or CPP-D
• Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, age, and/or gender identity
• Be knowledgeable about trauma-informed care, risk assessment, crisis intervention, and addiction/substance use
• Work collaboratively with partner agencies to promote services offered and establish referral pathways
• Work independently and collaboratively with a multidisciplinary team to carry out recovery and support
• Make appropriate external referrals as per the client centered case plan
• Keep up to date with knowledge of external community resources
• Understand and be able to integrate a philosophy of client-centered care into daily responsibilities
• Demonstrate a strong commitment to quality care and service delivery based on the HART Hub mission and values
• Ensure timely and appropriate records are kept and make sure that statistical information is maintained as per HART Hub standards
• Adhere to agency Health & Safety Program and the Client Safety Framework
• Perform additional duties as required/delegated by Management
Qualifications:
• MSW or BSW (2+ years experience) or SSW (4+ years experience) is required
• Membership in good standing with OCSWSSW
• Demonstrated case management, system navigation, and referral coordination experience required
• Current up-to-date local knowledge of Dufferin-Caledon community/health services and PHIPPA
• Criminal Records Check/Vulnerable Sector Screening
• Valid Ontario Driver’s License, valid insurance, and access to a reliable vehicle
Working Conditions and Requirements:
• This is an in-person, on-site, and in-community role
• Hours are 37.5 hours per week - including day and evening shifts, with some weekend hours
• Vehicle travel to sites and within community
