Crosstown Family Health Team (FHT) provides comprehensive primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients in its catchment area. The FHT employs an inter-professional team of health care providers who work in a collaborative model of care.

The Physiotherapist (PT) is critical to the effective functioning of the FHT. This position requires an ability to work in a fast-paced, changing environment and strong interpersonal skills to work with patients and inter-professional health care providers. This role will include addressing musculoskeletal concerns, vestibular concerns, pelvic floor health, neurologic issues, and chronic pain concerns.

This position will include 0.6 FTE with the Crosstown Family Health Team along with 0.4 FTE for private physiotherapy services with Crosstown Rehab.

Roles and Responsibilities:

Assess patients’ physical condition and medical history to identify concerns and develop treatment plans according to the College of Physiotherapists of Ontario guidelines and best practices.
Implement a variety of exercises, techniques, and interventions to assist patients in improving mobility, alleviating pain, and achieving their rehabilitation goals.
Providing patients with information regarding injury prevention and home exercise programs.
Monitor and track patients' progress throughout the treatment process while adjusting as needed.
Collaborate with other healthcare professionals to provide comprehensive care to patients.
Maintain accurate patient records and treatment documentation in accordance with professional standards and policies.Provide itinerant services working from different locations to create greater access to populations facing barriers to care.
Perform home visits, as required.
Maintains all licenses, certificates, and standards related to the ability to practice as a Physiotherapist in Ontario.
Related Responsibilities:

Collection of data as required for statistical/reporting purposes
Preparation of monthly and quarterly reports for Ontario Health
Participate in team building, program development, and improvement activities
Participate in staff meetings to exchange ideas, receive mutual support and monitor program outcomes
Comply with established personnel policies, procedures, standards and guidelines for operation
Qualifications and Requirements

Master of Science in Physiotherapy from a recognized university, or an equivalent combination of relevant education and experience.
Registered and in good standing with the College of Physiotherapists of Ontario.
Adequate malpractice insurance as required by the College of Physiotherapists of Ontario
Eligible for membership with the Canadian Physiotherapy Association.
Thorough knowledge of and proficiency in current physiotherapy best practices and skills.
Training in acupuncture is an asset
2 years of experience is an asset
Knowledge of community resources connected to the provision of health promotion and illness prevention
Skills in program planning, implementation and evaluation
Experience with Jane App, Telus Health eClaims, WSIB and MVA is an asset
Presentation skills and experience in group health education
Ability to maintain confidentiality
Ability to work in and adapt to a fast-paced environment
Ability to work independently and with a team
Ability to prioritize and manage time effectively in a changing environment
Must be able to work flexible hours, including evenings and possibly weekends
Experience working in Family Health Teams an asset, or any community health position (CHC, CCAC/OHH, etc) is an asset
Training and certification with pelvic physiotherapy is considered an asset.
Demonstrated ability in community health and public health sciences is an asset
Job Type: Full-Time ( 0.6 FTE of publicly funded practice with the Crosstown Family Health Team + 0.4 FTE for private physiotherapy services)
