This patient presented at the clinic today to receive their first dose of Astra Zeneca Covishield Covid19 vaccine. 
Their questions were answered and informed consent was obtained. 
Astra Zeneca vaccine was administered - 0.5ml IM «L»«R» deltoid without complications. 
Lot number and Expiry date are recorded in COVAXON system by Public Health.
The patient will be monitored for 15-30 minutes as per NACI guidelines and if stable, will be discharged from the clinic thereafter. 
No information was given regarding second dose appointment as this is not known at the time of vaccine administration.
