Summary/Abstract
The People's Health Care Act, 2019 introduced Ontario Health Teams, with the first coming in the fall. Teams are expected to offer a full and coordinated continuum of services, with preference for applications that include a minimum of hospital, home care, community care and primary care.
This panel presentation focuses on primary health care teams’ experience leading and participating in the OHT application process and what they’ve learned so far about this major change to the Ontario health system. More details to come.
