Attendees will hear about: 

  • a recently-concluded proof of concept demonstrating the clinical value of an EMR-integrated clinician dashboard   
  • the health quality improvement potential of dashboard-integrated health indicators that align with system priorities such as preventive care and opioid management 
  • findings and participant feedback illustrating how the dashboard drives improved clinician data entry, practice workflows and population health management 
  • the important role of hands-on clinician support and change management in realizing the dashboard’s full value to patient care.  
  • A demonstration will show how the dashboard tool can be quickly and easily integrated into practice workflows and used to enhance patient care.

Summary/Abstract

Primary care teams are amassing a wealth of data in their EMRs. They crave real-time access to that data at the point of care. They want to both view their patient data in aggregate and drill down to view individual patient data to identify population trends and individual at-risk patients for better preventive care. Current data quality offerings provide mostly outdated aggregate patient data, and don’t allow clinicians to drill down to patient level since they lack EMR integration.    

This presentation will focus on an OntarioMD Proof of Concept that developed an innovative EMR-integrated dashboard tool that:   

  • provides real-time visual representation of EMR data using widely-recognized primary care indicators;  
  • lets clinicians drill down to patient-level data for each indicator and take proactive steps to improve patient care;  
  • helps clinicians understand, and take steps to improve, the quality of patient data in their EMR;  
  • gives clinicians the option to trend and compare their indicator metrics with other dashboard users, offering a more complete picture of system-wide population health metrics; and  
  • can be offered provincially and expanded with new indicators that align with evolving data quality, practice and clinical priorities.    

Through the participation of 500 clinicians, the proof of concept demonstrated that the dashboard, combined with hands-on quality improvement support, has a measurable impact on clinicians’ ability to standardize and improve their EMR data entry, proactively run population health reports to identify at-risk patients, and identify and respond to patient heath needs during appointments. A province-wide rollout is currently being planned."""
