Job Title: Registered Nurse – Seniors Program Salary range: $39.51 – $41.93 per hour Reports to:

The NSFHT is a multi-disciplinary team located in Midland Ontario. We provide primary care,
education, and counselling services to patients in the North Simcoe Community. We offer a friendly,

Position Summary
The North Simcoe Family Health Team (NSFHT) is seeking a skilled and compassionate Registered Nurse
(RN) to join our Seniors Program. The RN will provide comprehensive nursing care to older adults,
supporting health maintenance, chronic disease management, and aging in place. Working within an
interdisciplinary primary care team, the RN will play a key role in care coordination, clinical
assessment, and patient and caregiver education.

Responsibilities and Job Expectations

• Conduct comprehensive nursing assessments for seniors, including physical health, functional
  status, cognition, frailty and falls risk.
• Monitor and refer to FHT programs for management of chronic conditions (e.g., diabetes,
  hypertension, COPD).
• Support medication management, reconciliation, and adherence in collaboration with prescribers
  and pharmacists.
• Provide health promotion, disease prevention, and self-management education to seniors and
  caregivers.
• Participate in care planning for complex and medically fragile seniors.
• Identify early signs of health decline and coordinate timely interventions.
• Collaborate with physicians, occupational therapists, nurse practitioners, social workers,
  clinical coordinator, and other allied health professionals.
• Participate in interdisciplinary case conference and Seniors Program Planning.
• Coordinate referrals to community supports and services (geriatric services).
• Maintain accurate, timely documentation in electronic medical records (Accuro).
• Contribute to quality improvement initiatives and population health activities.

Qualifications

• Baccalaureate of Science in Nursing.
• Current registration and in good standing with the College of Nurses of Ontario (CNO) as a
  Registered Nurse.
• Experience working with seniors in primary care, community health, geriatrics, or chronic
  disease management.
• Excellent communication and interpersonal skills.
• Knowledge of geriatric syndromes, frailty, polypharmacy, and aging-related conditions.
• Ability to work independently and as part of a team.
• BCLS.
  

Work Schedule:
This is a full-time, 12-month contract position with a standard 37.5-hour work week, Monday to
Friday.

Job Description

Nurse Educator, Registered Nurse

As a Nurse Educator, Registered Nurse (RN) within the Seniors Program of the North Simcoe Family
Health Team, you provide leadership in the planning, delivery, and evaluation of health education
initiatives for older adults and their caregivers. The role supports healthy aging, chronic disease
management, functional independence, and improved quality of life for seniors through
evidence-based, person-centered education.

Roles and Responsibilities:
Seniors-Focused Patient & Caregiver Education

• Develop, implement, and evaluate education programs for seniors and their caregivers addressing
  topics such as:
• Chronic disease management.
• Falls prevention screening and education.
• Mobility assessments.
• Medication management and polypharmacy.
• Cognitive health, dementia, delirium prevention.
• Mental health, social isolation, and caregiver stress.
• Advance care planning and end-of-life discussions.
• Deliver education using geriatric and adult learning principles, adapting content for sensory,
  cognitive and mobility needs.
• Promote self-management, functional independence, and health literacy among older
  adults.
  

Interprofessional & Staff Education

• Provide education and training to NSFHT staff on geriatric best practices, seniors care
  pathways, and age-friendly approaches.
• Support onboarding, mentorship and skill development related to seniors’ care.
• Collaborate with physicians, nurse practitioners, social workers, pharmacists, and allied health
  professionals to integrate education into clinical care.

Quality, Documentation & Compliance

• Maintain accurate, timely, and confidential documentation in accordance with NSFHT policies and
  PHIPA.
• Participates in data collection, reporting, and evaluation related to program outcomes and
  funder requirements.
• Adhere to standards of practice and ethical guidelines set by the College of Nurses of Ontario
  (CNO).
  Community Engagement & System Navigation
• Collaborate with community partners (e.g., home and community care, Specialized Geriatric
  Services) to coordinate education and referrals.
• Support transitions in care and promote awareness of community resources for seniors and
  caregivers.
  

Professional Practice & Leadership

• Practice in accordance with College of Nurses of Ontario (CNO) standards and NSFHT practices and
  policies.
• Demonstrate leadership in seniors’ health promotion, prevention, and education
  initiatives.
• Participate in committees, working groups, and evaluation activities related to senior’s
  care.
• Maintain competence in gerontological nursing through ongoing professional development.

Qualifications

• Current registration in good standing as a Registered Nurse (RN) with the College of Nurses of
  Ontario (CNO).
• Bachelor of Science in Nursing (BScN).
• Minimum 3-5 years of nursing experience, preferably in geriatrics, primary care, community
  health or chronic disease management.
• Demonstrated experience in patient, caregiver, or staff education.
• Strong knowledge of gerontological nursing principles and age-friendly care.
• Excellent communication facilitation, and interpersonal skills.
• Ability to work effectively within an interprofessional team and with complex
  populations.
  

Preferred/Assets

• Certification or coursework in Gerontology, Geriatric Nursing, Dementia Care, or Adult
  Education.
• Experience with quality improvement, program evaluation, and outcome measurement.
• Proficiency with electronic medical records (Accuro).
• Experience working in a Seniors Program or Family Health Team.
• Knowledge of community senior services supports and care coordination models.
