Job Title: Registered Nurse – Lung Health Program
Salary range: $39.51 – $41.93 per hour
Reports to: Clinical Director
Position Summary
The Registered Nurse – Lung Health Program provides comprehensive nursing care and education to
patients with chronic respiratory conditions within a primary care setting. The role focuses on
spirometry testing, lung health education, pulmonary rehabilitation, chronic disease management and
patient self-management strategies in collaboration with the interdisciplinary team.
Responsibilities
- Provide patient education related to lung health, respiratory disease prevention, inhaler
technique, smoking cessation, oxygen therapy and self-management strategies. - Support and facilitate pulmonary rehabilitation programming, including monitoring patient
progress and encouraging wellness goals. - Conduct nursing assessments and develop individualized care plans for patients with chronic
respiratory conditions such as COPD and asthma. - Act as a clinical resource for respiratory health and chronic disease management, while
monitoring patient symptoms, medication adherence, and treatment response in collaboration with
primary care providers. - Promote evidence-based chronic disease management and healthy lifestyle interventions.
- Maintain accurate and timely clinical documentation within the EMR according to professional
standards.

Qualifications
- Bachelor of Science in Nursing (BSN).
- Current registration and in good standing with the College of Nurses of Ontario (CNO).
- Minimum of 3 years of nursing experience, preferably in respiratory, chronic disease management,
or primary care settings. - Experience performing spirometry testing considered an asset.
- Certified Respiratory Educator (CRE) considered an asset.
- Certification or training in pulmonary rehabilitation considered an asset.

Work Schedule
This is a full-time, permanent position with a standard 37.5-hour work week, Monday to Friday.

Job Description
Nurse Educator, Registered Nurse
The Nurse Educator is a Registered Nurse who provides specialized patient and caregiver education
focused on lung health and chronic obstructive pulmonary disease (COPD) within the North Simcoe
Family Health Team. The Nurse Educator supports evidence-based education, self-management, and
quality improvement initiatives to improve respiratory health outcomes.
PRIMARY RESPONSIBILITIES:
Lung Health & COPD Education
- Deliver evidence-based education for patients with COPD and other chronic respiratory
conditions, including disease understanding, symptom recognition, and self-management strategies. - Provide education on inhaler technique, medication adherence, oxygen therapy, and device
use. - Support action plan development and early recognition of exacerbations.
- Facilitate group-based COPD education programs aligned with best-practice guidelines.
- Promote smoking cessation, vaccination, pulmonary rehabilitation, and lifestyle interventions to
improve lung health. - Spirometry testing.
- Assess learning needs, readiness, and health literacy of patients and families.
Clinical & Program Support - Collaborate with physicians, nurse practitioners, allied health professionals, and community
partners to support coordinated COPD care. - Collaborate closely with the Clinical Coordinator to support program development,
implementation, evaluation, and workflow optimization within the NSFHT. - Participate in respiratory-focused clinics and preventative care initiatives.
- Support standardized care pathways for COPD management within the NSFHT.
- Coordinate referrals to community and regional lung health resources.
Staff Education & Capacity Building
- Provide education and consultation to team members related to COPD best practices and patient
education strategies. - Coaching, and mentorship to NSFHT staff, students, and learner as appropriate.
- Support quality improvement (QI) and evidence-informed practice initiatives.
- Works in collaboration with interdisciplinary team to provide appropriate primary care safely
and effectively to patients.
Quality, Documentation & Compliance
- Maintain accurate, timely, and confidential documentation in accordance with NSFHT policies and
PHIPA. - Participates in data collection, reporting, and evaluation related to program outcomes and
funder requirements. - Adhere to standards of practice and ethical guidelines set by the College of Nurses of Ontario
(CNO).
Qualifications
- Registered Nurse in good standing with the College of Nurses of Ontario (CNO).
- Bachelor of Science in Nursing (BScN).
- Experience in respiratory care, COPD management, or chronic disease education.
- Certification or training related to COPD, asthma, lung health, or smoking cessation
(preferred). - Familiarity with COPD clinical guidelines (e.g., GOLD, Ontario Health pathways).
- Demonstrated experience in patient education and group facilitation.
- Strong communication, teaching, and interpersonal skills.
- Ability to work collaboratively with an interprofessional team.
- Proficiency with Accuro electronic medical records (EMR).
