Mental Health Therapist/Social Worker

Permanent | 30 Hours per Week

Minto-Mapleton Family Health Team (MMFHT) is seeking a Mental Health Therapist/Social Worker to
join our team. We are looking for a motivated self-starter who is friendly, organized, dependable,
and enjoys working with people in a collaborative healthcare environment.
Position Summary

As a member of our interdisciplinary primary care team, the Mental Health Therapist / Social Worker
provides assessment, treatment, and short-term counselling to patients experiencing mild to
moderate mental health concerns. The successful candidate will work collaboratively with family
physicians, nurse practitioners and other allied health professionals to support patients in
achieving their mental health and wellness goals.

Key Responsibilities include:

• Conducting mental health assessments and developing treatment plans
• Providing individual and group counselling using evidence-based approaches
• Supporting patients with self-management strategies and coping skills
• Providing system navigation and connecting patients with community resources and
  supports
• Collaborating with health care professionals and community partners
• Acting as a mental health resource for the healthcare team and broader community
• Maintaining accurate and timely clinical documentation within the electronic medical record
  (EMR)
  

Qualifications

• Bachelor or Master’s degree in social work or counselling psychology
• Registration and active membership with good standing with the Ontario College of Social Workers
  and Social Services Workers (OCSWSSW) or the College of Registered Psychotherapists of Ontario
  (CRPO)
• Demonstrated experience providing evidence-based interventions such as CBT, DBT, and
  trauma-informed care
• Experience working within an interdisciplinary primary care team is considered an asset
• Experience with electronic medical records is considered an asset
• Strong communication, interpersonal, organizational, and relationship-building skills
• Ability to work independently while contributing effectively as part of an interdisciplinary
  team
• Successful Criminal Record Check including Vulnerable Sector Screening.
  

Position Details
This position is available as a 0.8 FTE role (30 hours per week).

This position is primarily in-person at our MMFHT sites, with occasional virtual care delivery as
appropriate.
