Registered Nurse Regional Foot Care Program 1.0 FTE - Multiple Sites (1 year contract)

The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 100 physicians across 16 sites within London, and Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive inter-collaborative primary health care to over 155,000 patients. Over the past five years, our affiliated physicians have taken on more than 22,000 Ontarians who did not previously have a family doctor. Family Health Team staff includes nurse practitioners, nurses, social workers, pharmacists, dietitians and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. For more information about our organization, please visit www.thamesvalleyfht.ca. We are currently recruiting for a Registered Nurse (Foot Care Program). This 1.0 FTE contract position will provide foot care services at multiple sites requiring this service that do not currently have RN support dedicated to this function.  This unique position is best suited for an individual that works well independently and in a variety of settings. An appreciation for the aged patient is an asset as is an open mind to travel, and the ability to transition between different Electronic Medical Records and team cultures. The possession of organizational skills is imperative to the success of the candidate as is the physical capability to transfer equipment between sites when required. Preference will be given to candidates with the following:
  • Current registration with the College of Nurses of Ontario as a Registered Nurse
  • Experience in program development and group program delivery
  • Specialized training in providing foot care preferred
  • Knowledge and experience working collaboratively with community resources
  • Understanding of the Chronic Disease Management and Patient Self-Management Models
  • Proficiency in the use of computer and previous experience with an Electronic Medical Record
  • Must have own liability insurance
  • Ability to work within a multidisciplinary patient care team
  • A reliable personal vehicle suitable for the transport of required supplies and equipment
