Social Worker – West Durham FHT (Pickering)

An exciting opportunity is now available for a Full-Time Social Worker with the West Durham Family Health Team (WDFHT) in Pickering, Ontario.  Specifically, our mission is to provide comprehensive, interdisciplinary primary health care services, to our rostered patient population in the West Durham region.  A key focus of our interdisciplinary team is health promotion, prevention, early identification and support for chronic health conditions such as diabetes. We are searching for an energetic individual who is enthusiastic about becoming part of a progressive health care team.  As a key member of the WDFHT, the Social Worker will be part of an interdisciplinary team of health care providers including Physicians, Nurses, Nurse Practitioners, Dietitian, Social Worker and Pharmacist.  You will deliver services to a variety of patient populations with a focus on patient self-management, health promotion, disease prevention, chronic disease management. The Social Worker will participate in short-term individual and group counseling as well as help patients identify and use community health resources and agencies. Job Summary: The primary focus of the Social Worker is to restore, maintain and enhance the emotional and social functioning of a diverse population.  In addition, the Social Worker provides individual and group counseling, health promotion, and disease prevention, and care navigation to patients while supporting other health providers. Qualifications:
  • Master’s in Social Work
  • Current certification with the Ontario College of Social Workers, or eligibility for registration
  • Minimum 5-7 years experience as a Social Worker
  • Experience with CBT, Mindfulness and other solutions-based approaches is an asset
  • Excellent crisis intervention skills
  • Knowledge of community resources and familiarity with community outreach
  • Strong ability and interest in linking patients to community resources
  • Effective interpersonal and communication skills (written/verbal), self-directed.
  • Superior problem-solving and conflict resolution skills
  • A strong team player with the ability to set priorities and work effectively in a fast-paced, dynamic work environment. Effectively collaborate with members of an interdisciplinary team to plan, implement and coordinate patient care
  • Flexibility to accept changes in areas of responsibility and work environment.
  • Proficient in the use of computer and various software packages, including an Electronic Medical Record.
Roles and Responsibilities: The role and responsibilities of the Social Worker for the Family Health Team include but are not limited to:
  1. Assessment: Assessment and diagnosis of psychosocial problems
  2. Treatment and Management: Individual, couple, family and group counseling and psychotherapy; Case management, including linkages to community resources
  3. Education/Advocacy: Health promotion, psycho-education related to the prevention of mental health problems, assistance in navigating service delivery networks to find required resources and advocacy to establish and access needed resources.
