Job Description – Social Worker

0.5 Part Time (Permanent)    Employment (18.75 hours weekly)

Position Summary

As a key member of Connexion Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, you will deliver social worker care in accordance with FHT policies and professional practice standards. You will perform routine social worker duties, and ensure that the social needs of our patients are met. Your primary roles are to work in partnership with physician and other health providers to perform intake (via a semi-structured interview), patient psychosocial assessments, provide short-term counseling, education/advocacy and case management. You will deliver services to a variety of patient populations including high-risk groups suffering from chronic diseases and/or complex multi-system diseases. You will also provide health education to our patients with a focus on health promotion, disease prevention, and chronic disease management.

Skills, Knowledge Requirements

  • Strong verbal and written communication skills and the ability to adjust language and terminology to suit the individual patient or patient group
  • Strong clinical skills
  • Demonstrate cultural sensitivity
  • Autonomous and able to function with minimal supervision
  • Ability to work effectively with all members of the health care team
  • Ability to work in an inter-professional team environment
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
  • Familiarity with a Window OS environment
  • Experience with scheduling and electronic medical record keeping using a clinical management system is an asset
  • Bilingual (French and English) for verbal communication is required;
  • Minimum Education:  Masters Degree in Social Work
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Membership in the Ontario Association of Professional Social Workers
  • Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and community mental health resources
  • Adequate professional standard Independent Health practitioner insurance coverage
  • A minimum of 3 years of experience working in mental health and social services
  • Clinical experience working collaboratively with a community-based agency preferable
  • Teaching and experience facilitating treatment groups is an asset
