Registered Social Worker – OakMed FHT (Oakville)

Position Profile The OakMed Family Health Team (OakMedFHT) provides comprehensive interdisciplinary primary health care to patients and their families in the Halton Region. We are a proud supporter of community health initiatives, charitable ventures and we are an accredited teaching facility for Medical Residents and Allied Health Practitioners. We are currently seeking a dynamic full‐time Registered Social Worker to join our team who will work collaboratively with all members of our team by providing care for clinic patients and accepting referrals from, as well as consulting with the physicians as necessary. Provision of primary care will be within the scope of the Registered Social Worker (RSW) and she/he will play a leadership role in developing programs with the clinic to enhance patient care. Accountability The RSW reports to, and is responsible to, OakMedFHT Executive Director. All clinical decision making outside the scope of the RSWr must be done in consultation of the OakMedFHT Lead Physician and/or physicians/NP of OakMedFHT and/or medical directives. Authority The RSW will not direct the activities of staff or a function without direction of FHT decision makers. The RSW must comply with the standards of practice guidelines of the Ontario College of Social Workers and Social Service Workers. Decision Making On a regular and continuous basis, the RSW exercises clinical judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the FHT. Roles and Responsibilities The RSW is responsible for making clinical decisions, exercising judgement to ensure best possible care is provided within available resources. The RSW provides psychotherapy, counseling services, with particular regard to individual and family issues and decisions that impact the health of the patient and family. RSW has working knowledge and mastery in CBT, DBT, EFT, Interpersonal Psychotherapy, Systems, Single-Session, Narrative, Strengths-based and Solution focused therapies. RSW is able to work within the primary care model of brief therapy, ensuring seamless transition to community resources as appropriate; RSW is knowledgeable of the community and the resources available, especially in the areas of Crisis services, Mental Health, and Long Term Care. The RSW will assist the referring physician/health care providers with referrals to secondary and tertiary health care centers. The RSW will be enthusiastic, passionate and self-driven who will participate in data management and measurement of quality improvement indicators of patient outcomes. The RSW communicates results of measurements that are easy to use and understand, looking for patterns that could assist with improvement. The RSW acts as an advocate for patients and their families in matters affecting health, provides house calls and community outreach visits when appropriate and liaises with physicians and other team members as required. RSW to oversee family medicine residents and other health care professional students on the role of the Social Worker and psycho-social issues facing patients and families. The RSW must effectively use the Social Work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient. The RSW maintains own continuing education to enhance professional knowledge and growth and participates and collaborates with other Family Health Team members and health care providers in provision of patient care. The RSW acts as a professional and knowledgeable role model or mentor to colleagues and students; and maintains confidentiality in the work place and within the community. The RSW will be required to run evening groups (2-3 evenings) and day time groups. The RSW will be an independent thinker and problem solver, looking to challenge themselves and feels strongly about primary care. Qualifications Education & Specific Job Requirements
  • Masters Degree in Social Work;
  • Registered with the Ontario College of Social Workers and Social Service Workers;
  • Previous related patient care experience, specifically in a primary care setting preferred;
  • Minimum of 3 years’ experience;
  • Experience in CBT, DBT, EFT, Interpersonal Psychotherapy, Systems, Single-Session, Motivational
  • Interviewing, Mindfulness, Narrative, Strengths-based and Solution focused therapies solution focused
  • Ability to operate with the highest level of confidentiality and discretion;
  • Strong conflict resolution skills;
  • Exceptional oral, written and presentation skills;
  • Strong interpersonal skills;
  • Group facilitations skills;
  • In depth knowledge of chronic illness care, including both clinical and supportive care;
  • In depth knowledge of the local health program delivery and community based services;
  • Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population;
  • Ability to work effectively with all members of the health care team; and
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment.
