Registered Nurse - Thames Valley FHT (Woodstock)

Registered Nurse – 1.0 FTE (Woodstock, regular full time) The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 100 physicians across 16 sites within London, and Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive inter-collaborative primary health care to over 155,000 patients. The Family Health Team staff includes Nurse Practitioners, Nurses, Social workers, Pharmacists, Dietitians and other Health care Professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centered care. For more information about our organization, please visit Supporting the Alliance Family Health Organization team in Woodstock, the successful candidate will primarily provide navigational support to improve patient care in the community. PRIMARY RESPONSIBILITIES:
  • Provide health teaching, illness and injury prevention with clients and families on complex health care issues,
  • Raise the awareness of community services and build client and family capacity promoting client independence and self-management
  • Participate in home health nursing to support patients who may benefit from receiving service in the home, thus reducing visits to the ED and the primary care physician
  • Current registration with the College of Nurses of Ontario
  • Ability to provide episodic care under a medical order or directive
  • Knowledge and experience to coordinate patient care with community resources
  • Understanding of the Chronic Disease Management and Patient Self-Management Models
  • Demonstrated skills and experience in health promotion and preventative care
  • Proficiency in the use of computer and previous experience with an Electronic Medical Record
  • Must have own liability insurance, either personal or through RNAO
  • Ability to work in a multidisciplinary team to provide optimal patient care
  • Current Driver’s License and reliable vehicle
  • Experience in Home Health Care preferred
