Registered Nurse - Sunnybrook Academic FHT (Toronto)

The registered nurse (RN), working to his/her full scope of practice, provides professional nursing and collaborative care to patients and families of the Academic Family Health Team across the life span and health/illness continuum. Nursing care includes health promotion activities, disease prevention counseling and assisting the client with chronic disease management. Ensuring adherence to FHT policies, and nursing procedures, standards and practices, RNs are able to autonomously meet the nursing care needs of patients whose needs are not well defined/established or are changing.  RNs provide clinical support to the physician, clients’ follow-up for education and referrals, immunizations, dressing changes, telephone advice and triage and wound care.   RNs contribute to the education of health care learners. In this position the RN works with 3 experienced family physicians and is able to focus his/her care on those clinicians' practices. Roles and Responsibilities: Assessment
  •  Assesses the client’s health care needs (physical, emotional, psychological, and spiritual)
  • Initiates health assessments including a health history, identification of illness screening needs, immunization requirements/recommendations and under medical directives (i.e. healthy clients periodic health exam visits -perform screening maneuvers – PAPs)
  • Analyses the findings of a health assessment
  • Interprets client health records
  • Observes and records outcomes
  • Collects data through a therapeutic relationship with the client
  • In collaboration with the client and family and with other team members as appropriate, develops and discusses individualized care plans based on best practice
  • Monitors these plans with the client and family and assists them to revise the plan or to access the most appropriate care provider for updating
  • Provides nursing care and treatment (including complementary therapies and/or counseling) for health problems, illness prevention and chronic disease management
  • Documents using the electronic medical record and arranges follow-up as necessary
  • Helps clients to identify and use health resources
  • Involves clients in decisions about their own health
  • Encourages clients to take action for their own health
  • Initiates health education and other activities that assist, promote and support clients as they strive to achieve the highest possible level of health
  • Develops learning resources for clients, nurses and other health care providers
  • Actively participates in the education of health care learners (nurses, Family Medicine residents, clinical clerks and other allied health care providers)
  • Collaborates with other health care providers (care navigator, social worker, pharmacist, occupational therapist, diabetes team as appropriate)
  • Coordinates patient care and clinic flow
Operations/Organizational Role
  • Participates in the effective day-to-day functioning of the clinic
  • Contributes to FHT planning and operations by participating on various interprofessional committees
  • Participates in implementing organizational initiatives (i.e. Respect Program, Advanced Access, QIIP)
  • Assists in the orientation and development of new staff
  • Performs other associated duties as required
Skills, Knowledge Requirement:
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery with an emphasis on health promotion and chronic disease management
  • Ability to priorize and multi-task in a busy clinic environment (i.e. share appropriate information and teach while treating a patient)
  • Excellent verbal and written communications skills
  • Commitment to life-long learning
  • Bachelor of Science in Nursing with two years clinical experience (preferably in primary care)
  • License to practice nursing in Ontario through the College Nurses of Ontario as a Registered Nurse.
  • Membership in the Registered Nurses’ Association of Ontario, Ontario Family Practice Nurses’ Interest Group and evidence of liability insurance
  • Demonstrated advanced patient assessment and triage skills
  • Demonstrated experience working with seniors an asset
  • Knowledge of and working experience with computer systems and applications (previous experience with an electronic medical record preferred)
  • Excellent verbal and written communication skills.
  • Able to work collaboratively within an interprofessional environment as an active member of the FHT.
