Registered Nurse Patient Educator – NOTL FHT (Niagara-on-the-Lake)

Niagara-on-the-Lake Family Health Team One Full Time RN Patient Educator The Niagara-on-the-Lake Family Health Team (NOTL FHT) is currently seeking applications for one full time Registered Nurse Patient Educator to join our primary health care team. Successful candidates can expect to develop collaborative relationships with our 10 family physicians, specialists, dietitian, social work, nurse practitioners, fellow nurses, and support staff.  The RN will develop skills and knowledge in one or more key clinical programs i.e. hypertension, COPD, CHF, asthma, osteoporosis, falls prevention, and POC INR testing. Some hands on nursing will be expected as part of this role as well.  There may be some opportunity to develop new clinical programs based on the successful candidate’s passion and interests.  She/he will work collaboratively with all the noted disciplines to provide exemplary care to the patients of the NOTL FHT. Experience:
  • Preferred minimum 3-5 years experience in a primary health care setting.
  • Familiarity with the various primary care models In Ontario and the expectations from the Ministry of Health and Long Term Care a benefit.
  • Practical experience in conducting EKGs, spirometry, ambulatory blood pressure, administering vaccines and providing wound care an asset.
  • Previous participation in program development and community needs assessment.
  • Understand quality improvement  and the importance of data collection and analysis
  • Knowledge and experience with electronic medical records (NetMedical preferred)
Qualifications:
  • Current registration with the College of Nurses of Ontario.
  • Must be a team player, open to change and flexible in an evolving role.
  • Ability to work independently and co-operatively in a fast paced, multi-disciplinary team.
  • Proven dedication to patient education and patient empowerment for chronic illness self management.
  • Self-directed and patient focused.
  • Strong organizational, interpersonal and communication skills.
